An Environmental meeting was held for the above project on March 29, 2018, at 9:00 a.m. The meeting
was held at the project site. Those in attendance were as follows:

This meeting was held in accordance with the contract to discuss any environmental permits, permitted areas
on the plans and erosion control on the project.
Ms. Amy Euliss (Division 9 Environmental Supervisor II) open the meeting buy giving an overview of
the environmental permits and permitted areas on the plans. The project should be monitored every 7
days and within 24 hours of a'/z inch rain event. The Contractor will be responsible for the record
keeping up to the start of the 6 month permanent vegetation observation, and then the NC DOT will be
monitoring the project until the overall acceptance of the project. The website (MPE) will be use for the
monitoring of the rain fall. There are 2 monitoring locations for this project. The Contractor was
reminded of the corrective action time frames for the Routine and Urgent items on the NPDES forms and

the time frame for seeding and mulching. Any sediment loss into a wetland or juristically stream needs to
be reported to Ms. Stephanie Braquet or Ms. Amy Euliss within 24 hours along with any permit
modification request. All other sediment needs to be reported to Phil Suggs (Roadside Environmental
Field Operations Eng. (Div. 9& 10)) or Kat Marciniak (Roadside Environmental Field Operations
Technician) within 24 hours. The Northern Long Ear Bat moratorium will not be effect for this project.
The mortitorium for ICT#6 in the Contract will be omitted.
Mr. Bryan Reynolds and Mr. Dave Wanucha reviewed their respective permits. They both emphasized using
Best Practice Guidelines and reporting any loss of material.
Other items reviewed during the meeting were using native seeding, delineation of the project and to keep
updated erosion control plans on site.
