writing within ten (10) days after receipt of the data that you will not be using the electronic file or this disclaimer
stands as written. Refusing the terms stated above forfeits the right to utilize this electronic data.

This project is to construct the proposed road that will potentially serve the Middlesex Corporate Park. The footprints of
these actual facilities are not known at this time. The jurisdictional impacts associated with this project are the result of
constructing an adequate entrance road to access the usable land.

Take Highway 64 East out of Raleigh. Take the Middlesex / Hwy 561 exit towards Middlesex. Site is less than 0.10
miles on the left.
Please attach a map of site location using USGS 1:24,000 topographic map and the latest bound and
published County soil survey with the project/site boundaries delineated. A site plan should also be
included for all projects other than stream and wetland determinations.
