Enclosed please find a copy of the Pretreatment Compliance Inspection (PCI) form from the inspection
conducted on March 20, 2018. The PCI was conducted by Deborah Gore, PERCS Unit Supervisor, and
Johnathan Watts, Environmental Specialist, of the Fayetteville Regional Office. Mr. Brandon Holland, Mr.
Barry King, Mr. Bill Lester and Mr. Sam Stewart were present for the inspection and your time and
cooperation was much appreciated.
The purpose of the PCI is to determine the effectiveness of the pretreatment program by an in-depth
review of the files, including industrial monitoring data, appropriate data management and adherence
to the Enforcement Response Plan.
The Division of Water Resources (DWR) has determined that the Town's Pretreatment Program has failed
to meet the minimum requirements as specified in 15A NCAC 2H .0900 and 40 CFR 403.12 (e). Specifically:
1. The Town has failed to require monitoring by Grede II, Biscoe LLC in accordance with the
Industrial User Permit (IUP);
2. The Town has failed to perform the POTW required monitoring and inspections at Grede
II, Biscoe LLC and at Flakeboard America Limited.
3. Failed to take appropriate enforcement action for limits violations at Carolina Dairy.
To avoid further enforcement action, perform the following actions and respond by the indicated due
date.
1. An Industrial User Permit (IUP) has been issued to Grede LL, Bisoce LLC, a foundry/casting
manufacturer. Take the following actions no later than April 30, 2018:
a. Inspect Grede and determine if process wastewater is being discharged. They have
identified themselves as 464, Metal Molding and Casting.

b. If process wastewater is being discharged, determine what operations under Subpart C
— Ferrous Casting are discharging (see attached copy of 464.35) and request data on
amounts of metal poured. Contact PERCS for assistance in determining permit limits.
c. If no process wastewater is being discharged you may drop the IUP.
d. If the IUP is not dropped prior to March 31, 2018, Grede is responsible for quarterly
monitoring as per the IUP.
e. Issue NOV to Grede for failure to monitor in 2017 and include them in the Public Notice
of Significant Non -Compliance. (see March 26, 2018 PAR correspondence)
2. An Industrial User Permit (IUP) has been issued to Flakeboard America Limited, a high-pressure
laminate manufacturer.
a Inspect Flakeboard no later than May 31, 2018.
b. Process wastewater is thought to be hauled off -site for disposal. If the inspection
confirms this the Town may decide to drop the IUP. It the Town does not drop the IUP
the Town must perform sampling by June 30, 2018.
3. Issue NOVs to Carolina Dairy for BOD violations in 2017 and include them in the Public Notice of
Significant Non -Compliance. (see March 26, 2018 PAR correspondence)
4. A site -specific HWA is needed. Implement your monitoring plan beginning in the 2nd quarter of
2018 and collect 4 quarterly samples ending with the 1st quarter of 2019. The site -specific HWA
will be due August 1, 2019.
The inspection report is attached with findings and observations made during the inspection. Please
review for more detail.

follow SUO entry procedures.

Requirements:
1. Perform on -site inspection to determine compliance status of Grede and Flakeboard.
2. Issue NOV's to Carolina Dairy, Grede/American Axle, and Flakeboard America per the Enforcement Response Plan (ERP) and
issue a public notice of significant non-compliance in the local newspaper.
3. Grede/American Axle: It is unclear if any process wastewater is discharged to the sanitary sewer. Confirm how process
wastewater is disposed of and take appropriate action — issue a permit containing the categorical requirements or rescind permit as not
necessary. Grede is responsible for quarterly sampling if IUP is not dropped prior to March 31, 2018.
4. Flakeboard/Aruaco: Rescind the permit before May 31, 2018 or commence sampling by June 30, 2018.
5. Modify permit to revise daily limit on BOD for Carolina Dairy or place the SIU under a Consent Order
