EXPRESS REVIEW ACCEPTANCE LETTER

On January 10, 2007, the Wetlands/401 Unit of the Division of Water Quality received a Fax/email
request from Kevin Yates, John R. McAdams, Inc., regarding a modification to a project known as
"Merck Vaccine Manufacturing Facility stormwater management plan" for acceptance into the Express
Review Program. This letter advises you that your project will be accepted into the Express Review
Program once the following items are received:
a. The application fee of 500.00 made payable to the North Carolina Division of Water Quality;
b. Three (3) complete and collated copies of the Express Review Stormwater Management Plan
Application [http://h2o.enr.state.nc.us/ncwetlands/dacumentslExp Rev SWMA pdfl;
c. Three (3) copies of all supporting information pertaining to this project (site development
plans, stormwater management plans, worksheets, etc.);
d. One (1) original and two (2) copies of an Operation & Maintenance Agreement for the
stormwater management facilities.
