PART 1
GENERAL PROGRAM INFORMATION
The City of High Point's Pretreatment Program monitored twenty-four significant
industrial users for most of 2017. The pretreatment program renewed three SIU
discharge permits and modified two SIU permits during 2017. All significant industrial
users had at least one significant industrial user facility inspection during 2017 and
the pretreatment program collected compliance samples in both the spring and fall.
There were no new SIUs added to the program but Seaboard Group II was dropped
from the Pretreatment Program in August.
Headworks analyses for both plants were completed in 2017; however, due to the
delay in the Westside upgrade project, their headworks analysis will be re -calculated
using data from a short-term monitoring plan after one year of full operation. The
short-term and updated long-term monitoring plan is due in June 2018. The long-
term monitoring plan for Eastside is also being modified at the request of the State
due to discrepancies in detection limits used. It is also due in June 2018.
The new headworks calculations for the Westside treatment plant rectified an over-
allocation that has existed since 2012. Additionally, it was determined the need to
modify existing local limits. The request has been submitted and the issue is
scheduled for City Council approval in early 2018. Once approved, the Sewer
Ordinance will be updated and submitted to NC DEQ PERCS.
Eastside Wastewater Treatment Plant (WWTP)
There were no upsets, interferences, or pass through incidents confirmed to have
been attributed to currently permitted industrial users at the Eastside WWTP.

The incinerator remains under construction and the comingled sludge is being hauled
for compost by McGill Compost Company under their biosolids permit. The Eastside
WWTP existing NPDES permit expires on December 31, 2018.
Westside WWTP
The Westside WWTP is currently in the process of a plant expansion/upgrade
project which was expected to be completed in Spring 2017; however, the completion
date has been delayed until Spring 2018. The Westside WWTP is currently operating
with an expired NPDES permit; the previous permit expired on April 30, 2014. The
dates listed for the Headworks Analysis (HWA), Long Term Monitoring Plan (LTMP),
Industrial Waste Survey (IWS), Sewer Use Ordinance (SUO), and Enforcement
Response Plan (ERP) are all correct.
Corrections to Information Sheet
The Eastside SIU permitted flow and percentage needs to be corrected. Currently
the information sheet indicates the Eastside permitted SIU flow is 1.156 which
equates to 4.45 %; however, permitted significant industrial users account for a total
flow of 1.192 MGD which equals 4.58 % of Eastside's design flow. The NPDES effective
date for Westside is incorrect. The permit the Westside plant is operating under was
effective on August 1, 2013 and was only effective until April 2014. Additionally, Ms.
Alannah Russell should be added the pretreatment contact list, she is a new
laboratory analyst assigned to Pretreatment. Her contact information has been added
to the Eastside information sheet. The flow changes are highlighted and noted in red
text on the program information sheets which are included on the following pages.

SIU INFORMATION AND INSPECTION SUMMARY
All significant users had at least one significant industrial user facility inspection
during 2017. A summary of their information and inspections are listed below:
WESTSIDE WWTP, NPDES Permit Number NCO024228
There were four (4) industries recognized as significant contributors during 2017 who
meet one or more of the following criteria:
I. Average Daily Flow 25,000 gallons or more.
II. Capability to significantly impact the POTW.
III. Subject to National Pretreatment Standards.
IV. Subject to Local Pretreatment Limits.
SIU SUMMARY
Cascade Die Casting Producer of custom die cast and machined
1800 Albertson Rd aluminum castings; they are subject to 40
High Point, NC 27261 CFR 464.16. They report that their business
production is similar to 2016. New tool
building construction should be done in
January. There were no violations noted
during the inspection.
2. Dairy Fresh, Inc. This facility is a dairy processor. They are not
1350 W Fairfield Rd subject to categorical regulations but are
High Point, NC 27263 classified as a significant user due to flow
greater than 25,000 gallons per day. Their
production has increased significantly
compared to 2016. There were no violations
noted during the inspection.

Specialty chemical manufacturer. They are
subject to 40 CFR 414 D (OCPSF). Their
production has decreased by 3 to 4%
compared to 2016. Currently troubleshooting
causes of recent BOD/TSS violations. There
were no violations noted during the
inspection.
Specialty chemical manufacturer. They are
regulated under 40 CFR 414 H (OCPSF). They
report their production volume has increased
compared to 2016. There were no violations or
recommendations made during the
inspection.

SUMMARY
Producer of polyurethanes and
polyacrylates. This industry is regulated
under 40 CFR Part 414 G & H. Production
has increased compared to 2016. There
were no violations noted during the
inspection.
This SIU is an industrial laundry that is not
subject to categorical regulations but
considered a significant user because daily
flow is greater than 25,000 gallons per day.
They report their business is increased
compared to last year. There were no
violations noted during the inspection.
This facility reclaims steel drums and is not
subject to categorical regulations and
considered to be a significant industry due
to the potential for causing adverse effects
to the POTW. Business has increased
compared to 2016 to point of adding
additional workday. Discussed possible
causes for recent copper, lead and zinc
violations. Made recommendations for
staging cleaned drums and "to be
processed" drums. Noted the label missing
from drum containing parts washer rinse
water. No other deficiencies observed.

4. Diversified Technologies
This CIU is a printed circuit board manufacturer

5. Fairmont Metal Finishing
This CIU is regulated under 40 CFR 433 A,

discussed their plan to correct them.

This textile facility is not subject to categorical

They reported their production was down
compared to 2016 and at end of year will lose
major contract. Added sock printing this year
which does not generate wastewater. At the time
of inspection, they were in the process of
replacing drums and overhead piping in the dye
house area. There were no violations noted
during inspection.
7. Hunter Farms
This facility is a dairy processor. They are not

Significant because their discharge flow exceeds
25,000 gallons per day. They report their
production is similar to 2016. Despite
improvements to pretreatment system in 2016
they have had several bypass incidents during
2016; a show -cause hearing scheduled for Dec.
has been continued to early 2018. There were
deficiencies with the discharge pH recorded on
their daily logs, with pH being outside their
permitted limits frequently during overnight
hours, which is abnormal, provided their
equalization.

8. Innospec Performance Chemicals

SUMMARY
This CIU is an organic chemical manufacturer.
They are regulated under 40 CFR 414 G & H.
Production has increased 17% compared to
2016. There were no violations noted during the
inspection. Will be sending updated SPCC plan
once CAD drawing complete.
9. Kersey Valley Landfill
This facility is a landfill and not subject to

categorical regulations and considered to be a
High Point, NC 27260
significant industry due to the potential for
causing adverse effects to the POTW. The new
pump station came online in March. No
violations noted during the inspection. They
were reminded about annual meter calibration
requirement and provided a list of calibration
contractors.
10. M.B. Plating
This CIU is subject to regulation under 40 CFR
1301 Corporation Drive
433, metal finishing. They share a building with
High Point, NC 27263
Fairmont Metal finishing. They are no longer
plating any metal. They perform black oxide
coating which does not produce any wastewater
to discharge. Deficiencies identified by recent
RCRA inspection were reviewed and discussed
their plan to correct them. Also discussed the
possibility of closing the business.
11. Mickey Truck Bodies
This facility is a CIU subject to regulation under

40 CFR 433 Subpart A. This facility reported a

significant increase in production compared to
2016. They will be adding additional employees
for a full second shift for 2018. No violations
were noted during the inspection.
12. Patheon Softgels, Inc.
This facility produces gel -coated pharmaceutical

and nutritional supplements (aka Over -the -

Counter Medications or OTCs) and subject to
regulation under 40 CFR 439 Subpart D. They
are now operating under a new parent company,
Thermo Scientific. They reported their overall
production has decreased compared to 2016.
The plans to replace the pretreatment building
have been delayed. There were no violations
noted during the inspection.

SUMMARY
This textile mill is not subject to categorical
regulations and considered significant due to
discharge flow exceeds 25,000 gallons per day.
This location is an overflow of production that
occurs at their Centennial Street location. They
have acquired two adjacent buildings, adding a
total of 80,000 ft2. to expand their dry
operations. In the planning stages to re -locate
sewer line and flume. There were no violations
noted during the inspection.
15. Slane Hosiery This textile mill is not subject to categorical
313 S Centennial Street regulations and considered significant due to
High Point, NC 27260 discharge flow exceeds 25,000 gallons per day.
They reported their production is up from 2016.
No violations were noted during the inspection.
16. SYNTEC Seating Solutions, LLC

17. Teknor Apex - North Carolina

This CIU is subject to categorical regulation
under 40 CFR 433 subpart A, metal finishing.
This facility fabricates seats for the
transportation industry with Thomas Built
Buses being their primary customer. SYNTEC
shapes, welds, phosphate -coats and powder
coats metal parts. They report an increase in
production compared to 2016. No violations
were noted during their inspection.
This facility is regulated under Part 414,
Subpart D. They manufacture thermoplastic
polymer pellets. They indicated a slight increase
in production rates compared to 2016. They
changed all extruders to waterless vacuum
system to prevent volatile organics leaching into
the wastewater. As a result, their water usage
and subsequent discharge has dropped
dramatically, from an estimated 30,000 gallons
per day to 3,000 gallons per day. There were no
violations noted during the inspection.

18. Thomas Built Buses
This CIU is subject to regulation under 40 CFR
1408 Courtesy Road
433, metal finishing. Production is up 5%
High Point, NC 27260
compared to last year. As of January 2011 they
are landfill free and have achieved Steward level
with NC Environmental Stewardship and are
currently working on de -listing their sludge.
Upgrades to the pretreatment system are
planned and will take place when the plant is
shut down. There were no violations noted
during the inspection.
19. Ultra Coatings
This CIU is subject to regulation under 40 CFR
3509 Jamac Road
433, metal finishing. Their process includes
High Point, NC 27260
cleaning, phosphatizing and powder coating
metal objects. They are a job shop for
companies such as Honda and John Deere and
report no changes in their production rate but
are expecting an increase in 2018 which will
impact wastewater flow. They will contact the
City for permit modification when appropriate.
There were no violations noted during the
inspection.
20. Zebra Environmental Services

This facility is categorically regulated under 40
CFR 437 B, Central Waste Treatment for oily
wastes. They dispose recovered groundwater
contaminated with gasoline or diesel fuel after
phase separation. They report business is
steady, similar to 2016. There were no violations
observed during the inspection.

There were no new significant industrial users added to the pretreatment program
in 2017 that discharge to the Eastside WWTP; however, Seaboard Group II was
dropped from the pretreatment program in August 2017. They began the
phytore mediation operations in March; however, the decision was made not to have
the option to discharge to the wastewater plant. Therefore, the discharge pipes were
capped and valves locked. The keys for the valve locks are in the possession of the lab
and pretreatment division.

ENFORCEMENT ACTIVITY: There were no enforcement activities in 2017.
INDUSTRY: Dairy Fresh, Inc.
NATURE OF VIOLATION: BOD and TSS
SUMMARY: Dairy Fresh, Inc. was issued a Notice of Non -Compliance in March,
September, and November for exceeding their daily BOD limit. In May they received a
Notice of Violation and fined $250.00 for exceeding their daily TSS TRC limit. In July,
they received a Notice of Non -Compliance for a daily TSS violation and in December
they received Notices of Violation and a fine of $250.00 for exceeding their daily BOD
limit and daily TSS TRC limit.
INDUSTRY: KAO Specialties Americas, LLC
NATURE OF VIOLATION: BOD and TSS
SUMMARY: KAO Specialties Americas, LLC (KSA) was issued a Notice of Non -
Compliance in June, October, and November for daily BOD limit violations and in
August and September for daily TSS violations. They had several occasions in which
they exceeded their daily max flow volume due to rainfall but their permit stipulates
these types of exceedances are allowed.

SUMMARY: Custom Drum received five Notices of Violations during the month of

December but at their lawyer's request, was postponed until after January 1st. The

SIGNIFICANT NON-COMPLIANCE SUMMARY
The City of High Point had one industry in Significant Non -Compliance (SNC)
during 2017:
MB Plating
MB Plating was in Significant Non -Compliance for both the first half and second
half of 2017 for reporting as defined in 15 NCAC 02H .0903(35)(F) and as adopted in
the City of High Point Sewer Use Ordinance. MB Plating failed to submit their
monthly self-monitoring reports for January, February and March. As noted in
section 3, escalating enforcement was not initiated considering the owner's
unexpected passing in December and uncertainty of the business' future.
The City of High Point is required to make public notification of industrial waste
contributors that have been in Significant Non -Compliance with Categorical
Pretreatment Standards. That obligation was fulfilled on February 13, 2018 in the
High Point Enterprise. A copy of the public notice is included on the following page.

MISSING DATA & DATA EXPLANATION SUMMARY
Kersey Valley Landfill
The landfill pump station went online in March. When the pump station is online, no
discharges are expected from the pond or the tank as all leachate is routed to the
pump station; however, when the pump station went online, there was still leachate in
the tank. In order to prevent septic conditions, they pumped the remaining contents
and given instructions to collect an additional sample.
MB Platin
MB Plating did not submit self-monitoring reports for January, February or March.
The data summary for the first six months is based on the POTW sample only. Due to
decreased operations, their permit was modified which omitted the monthly self-
monitoring requirement for the second six-month period; however, no discharges
occurred.

Our current local limits are applicable to both the Westside and the Eastside
wastewater treatment plants; therefore, the need to modify them will not be evaluated
until after Eastside's headworks is calculated in the upcoming months. The more
stringent of the two will be compared to the current local limits and if necessary, will
be updated.

Enclosed is the 2017 Headworks Analysis for the Eastside wastewater treatment plant.
The submittal contains two bound books, one that includes all data and calculations
for the HWA and a second that is the DMRs, process control spreadsheets and SIU
data summaries.
Our current local limits are applicable to both the Westside and the Eastside
wastewater treatment plants; therefore, the need to modify them is currently being
evaluated. Once both headworks analyses are approved, the more stringent of the two
will be compared to the current local limits and if necessary, will be updated.

permit modification. The landfill permit mimics the previous permit with an added exception
for exceeding daily maximum flow during documented high rainfall events. Copies of their
permit application and most recent facility inspection were submitted via electronic mail on
June 1, 2017 and are not included with this permit packet.
Innospec Active Chemicals' permit modification also includes an exception for exceeding daily
maximum flow during high rainfall events due to roof surface drains discharging through their
pretreatment system.
Both permits require notification, as with other violations, within 24 hours of being aware of

The Pretreatment, Emergency Response, and Collection Systems (PERCS) Unit of the Division of Water
Resources has reviewed the Headworks Analysis (HWA) for the Westside Wastewater Treatment Plant
(WS WWTP) submitted by the City of High Point. The HWA was received on April 27, 2017.
The Division concurs with the HWA calculations for all pollutants of concern, with the corrections and
observation outlined below. These approved Maximum Allowable Headworks Loadings (MAHLs),
Maximum Allowable Industrial Loadings (MAILS), and the basis for these values are found on the last page
of the enclosed HWA spreadsheet, which has the corrections marked. The results of the HWA are also
listed in the bottom section of the enclosed Allocation Table.
The following observations were noted during the review of the HWA:
1) Changes to the HWA Approved Tom: Please feel free to contact PERCS if you have any questions
about the changes outlined below, or would like to discuss possible alternatives. All corrected
spreadsheets are attached, and electronic versions were also emailed to you for your files. Please
ensure the POTW's paper and electronic HWA, AT, and other supporting documents contain these
corrected documents, especially the Allocation Table. As needed, please mark any original documents
as "replaced" or "voided."
a) Allocation Table: Allocation Table was updated in accordance with the most recent IUP renewals
as of July 1, 2016. Please use this updated AT for your future references.
2) Detection Levels: it is noticed that LTMP/DMR sampling data for several parameters during HWA
sampling period did not meet LTMP approved detection levels (DLS)/PQLs as well as overall DLs were

3) HWA Due Date: The HWA approved today is based on DMR and LTMP data for the period January
2014 through December 2016. As mentioned by the POTW, the Westside WWTP is in the middle of
expansion and expected to be completed by the end of March 2018. Please notify PERCS if there are
any changes of the proposed completion date. POTW must need to perform the following tasks:
a) Submit for approval updated LTMP by June 1, 2018.
b) After the upgraded WWTP is out of start-up mode (usually six-month), start monthly influent and
effluent samplings for the next 12 months, starting September 2018 through August 2019.
c) Submit site-specific updated HWA by December 1, 2019.

on July 26, 2017 that this facility had terminated the discharge connection. The discharge pipe
was removed and plugged at both ends and subsequent valves were locked and the key is now
in my possession.

Notice of Intent to Issue Civil Penalties
Dear Mr. Moore,
NORTH CAROLINA'S LNTERNATIONAL CITY
The City of High Point is required by federal law to operate and enforce an Industrial Wastewater Pretreatment Program
whose purpose is to prevent industrial discharges to the sewer system that have the potential to cause damage to the
collection system, the treatment facility or to the environment. Please be advised that the City of High Point has
determined that Hunter Farms failed to comply with certain sections of its Industrial User Permit #0107. The City's
determination is based on information obtained from investigating the incidents described below.
As you are aware, the City of High Point responded to a sewer back up and overflow incident behind your facility in the
early morning hours of May 18, 2017. In addition to causing a sanitary sewer overflow, the incident caused significant
damage to the private residence at 1920 Montgomery Street, which resulted in a considerable restoration expense to the
City.
A follow up inspection was performed on May 18, 2017 at approximately 10:00 am. The following conditions were
observed:
+ The Effluent flow meter chart & pH recorder were out of service.
+ On-site lift station revealed evidence that the water level was at or above the pipe level, which would
allow untreated wastewater to exit through the flume, bypassing both pH neutralization, solids settling
and the Dissolved Air Floatation (DAF) unit.
+ Heavy fats, oil & grease (FOG) observed in manhole directly downstream of on-site lift station.
+ Evidence that the water line was approximately 6" above the level of the flume, which indicates the flume
at one point, was overflowing.
+ The manhole in front of 1920 Montgomery St. had evidence of heavy FOG on the walls and the depth of
the manhole.

Based on the inspection findings for the May 181h incident, it has been determined that Hunter Farms violated the
following permit conditions:
IUP #0107: Part ll, Item #7: Facilities Operation, Bypass — The permittee shall at all times maintain in good
working order and operate as efficiently as possible, all control facilities or systems installed or used by the
permittee to achieve compliance with the terms and conditions of this permit. Bypass of treatment facilities is
prohibited except when approved in advance by the Control Authority and/or Municipality. Bypass approval shall
be given only when such bypass is in compliance with 40 CFR 403.17.
® Hunter Farms did not have approval to bypass treatment facilities nor did they notify the City of the
bypass condition within 24 hours as required.
•:• IUP#0107: Part Ill, Item #3: Flow Measurement Requirements - The permittee shall maintain appropriate
discharge flow measurement devices and methods consistent with approved scientific practices to ensure the
accuracy and reliability of measurements of the volume of monitored discharges. Devices installed shall be a
continuous recording flow meter capable of measuring flows with a maximum deviation of less than 105vofrom true
discharge rates throughout the range of expected discharge volumes. The totalizer shall be non-resetable. The
devices shall be installed, calibrated, and maintained to ensure accuracy. At the time of issuance of the permit, this
method consists of non-descript flow meter. The meter shall be calibrated every year, and the permittee shall
include a copy of the calibration report with the applicable monthly report required under part Il, 2, of this
permit. Modifications to the flow metering equipment shall be approved by the Control Authority prior to
installation. If a required flow measurement device fails, the Control Authority shall be notified within 24 hours.
Hunter Farms did not notify the City of High Point Lab/IPP Division that their effluent meter chart and
pH recorder were out of service.
IUP#0107: Part ll, Item #30: Potential Problems — The permittee shall provide protection from accidental and slug
discharges of prohibited materials and other substances regulated by this permit. The permittee shall also notify
the POTW immediately of any changes at its facility affecting the potential for spills and other accidental
discharge, discharge of a non -routine, episodic nature, a non -customary batch discharge, or a slug load as defined
in the Sewer Use Ordinance.
Additionally, the permittee shall notify by telephone the Control Authority and/or Municipality immediately of all
discharges that could cause problems to the POTW including any slug loadings as defined in the Sewer Use
Ordinance. If the permittee experiences such a discharge, they shall inform the Control Authority and/or
Municipality immediately upon the first awareness of the commencement of the discharge. Notification shall
include location of the discharge, type of waste, concentration and volume if known and corrective actions taken
by the permittee. A written follow-up report thereof shall be filed by the permittee within five (5) days, unless
waived by the Control Authority and/or Municipality.
• The pipe connected to the on-site lift station provides an opportunity to bypass treatment; therefore,
does not protect the POTW (collection system) from accidental, non -routine and/or slug discharges.
• Hunter Farms did not notify the City of High Point a bypass situation had occurred.
A meeting between Hunter Farms and City of High Point personnel occurred on May 29, 2017 to discuss the overflow
incident, treatment questions and storage capacity. The cause of the bypass was reported to be caused by pump failure.
When and the argument was made that if the "bypass" pipe was removed or plugged, there is potential for overflows onto
the property and adjacent parking lot. Key pretreatment and maintenance staff were absent from the meeting that could

provide answers to most of the City's questions; therefore, responses to the following questions were provided
approximately two weeks later:
1. What is the pump capacity (gpm) of the pit (on-site lift station) and how do they operate?
2. Is there a bypass alarm system on the lift station? If so, is it operating correctly?
3. Is it possible for Hunter Farms to contact the City when maintenance occurs?
In regard to Question #1:
Both pumps are rated for 250 gpm each and would total 400 gpm minus back pressure and work off a level probe.
Pump #1 activates when wastewater level in the pit reaches 38" and shuts off when level drops to 15'; pump #2 comes on
when wastewater level reaches 60" and shuts off at 30".
The response to Question #2:
There is a high-level alarm which includes a red light and siren sound that activates when the wastewater level
reaches 70'; which is approximately 3 feet below the bypass pipe.
The following protocol was implemented in regard to Question #3:
Wastewater operators were instructed to call the City of High Point Customer Service number to notify Sewer
Mains and Industrial Pretreatment about the bypass. As part of the notification, they must give the nature of the bypass
and expected duration. They will also notify the City of High Point when they have routine jetting maintenance performed
on their sewer lines. Signs have been put on the lift station pump stations and main control panel. Additionally, security
was notified to contact maintenance personnel if they hear the bypass alarm.
There were three additional bypass incidents following the bypass protocol implementation. The first bypass incident
occurred on June 29t'', the second bypass incident was on July 6th and the third incident occurred on September 14, 2017.
A separate hazardous material spill occurred on July 19th and is unrelated to wastewater bypass occurrences. For all three
bypass incidents, Hunter Farms staff followed the internal protocol and notified the City, who responded to circumvent
any sewer issues. The bypass incidents were reportedly triggered by excess flow from the main plant during CIP
operations. The spill incident commanded fire department, sewer mains and stormwater response. The spill entered on
Main St., and subsequently into the storm drain system. While not a sanitary sewer issue, the spill incident does
emphasize an observed pattern:
1. The accidental discharges are occurring at similar times of the evening
2. The discharges involved CIP process wastewater
3. The incidents all occurred on Thursdays
Enforcement
The City of High Point is prepared to impose the following enforcement actions:
May 18, 2017 sewer overflow incident:
Y Issue Notice of Violations for failing to comply with the following Permit Conditions: Part II, Item #'s 7, 30 and
Part III, Item #3.

• Pursuant to the City of High Point Enforcement Response Plan, violation of permit conditions may result in
penalties up to $25,000 per day, per violation. Based on the severity of damage caused by the bypass and/or
subsequent slug discharge, Hunter Farms is responsible for reimbursing the city its mitigation and repair costs in
the amount of $59,749.74.
• Consider the facility in Significant Non -Compliance for the first half of 2017 as defined in 40 CFR Part 403.8
(f)(2)(viii)(C) and as adopted by the City of High Point Sewer Use Ordinance:
Any other violation of a Pretreatment Standard or Requirement as defined by 40 CFR Part 403.3(1) (daily
maximum, long-term average, instantaneous limit, or narrative standard) that the Control Authority
and/or POTW determines has caused, alone or in combination with other discharges, interference or pass
through (including endangering the health of POTW personnel or the general public).
June 29, 2017 Bypass incident:
• Issue a Notice of Violation for violating Permit Condition Part II, Item #7.
• Issue a civil penalty in the amount of $662.03.
July 6, 2017 Bypass incident:
• Issue a Notice of Violation for violating Permit Condition Part II, Item #7.
• Issue a civil penalty in the amount of $537.27.
September 14, 2017 Bypass Incident:
• Issue a Notice of Violation for violating Permit Condition Part II, Item #7.
• Issue a civil penalty in the amount of $1,422.33.
Based on the accumulative incidents, Hunter Farms shall submit within 90 days, a compliance response describing
measures implemented or plan to implement to circumvent bypass occurrences and address the following items:
1. Evaluate whether staggering the CIP cycles is possible so that CIP wastewater from all fixtures are not discharged
to the pretreatment system at the same time.
2. Add additional equalization capacity. The lack of storage is a contributing factor to slug discharges and hydraulic
surging, leading to partial treatment in the DAF unit.
3. Increase the size of the DAF unit. The City of High Point has reviewed the initial pilot testing data which indicates
the DAF design is based on 120,000 gallons per day. Current operations, with average daily flows approximately
170,000 gallons per day, exceed the original design specifications of the existing DAF unit.
4. Staff a dedicated wastewater operator for all shifts, preferably certified as a Physical -Chemical Wastewater
Operator,

Hunter Farms has the opportunity to show cause as to why the City should not proceed with the outlined enforcement
actions, as described in City of High Point Sewer Use Ordinance Section 8-2-60.1(C):
Show cause hearing- The POTW Director may order any industrial user who causes or is responsible for an
unauthorized discharge, has violated this article or is in noncompliance with a wastewater contributor permit to
show cause why a proposed enforcement action should not be taken. A notice shall be served on the user
specifying the time and place for the hearing, the proposed enforcement action, the reasons for such action, and a
request that the user show cause why this proposed enforcement action should not be taken. The notice of the
hearing shall be served personally or by registered or certified mail (return receipt requested) at least 10 days
before the hearing. Service may be made on any agent or officer of a corporation.
The POTW director shall review the evidence presented at the hearing and determine whether the proposed
enforcement action is appropriate. A show cause hearing under this section is not a prerequisite to the assessment
of a civil penalty under subsection 8-2-60 nor is any action or inaction taken by the POTW director under this
section subject to an administrative appeal under subsection 8-2-56.2 POTW director's final decision shall be to
either proceed with the proposed enforcement action or to modify the action. Such modification may include but is
not limited to the issuance of an order to the industrial user directing that, following a specified time period, the
sewer service be discontinued unless facilities, devices or other related appurtenance shall have been installed and
are properly operated. Further orders and directives as are necessary and appropriate may be issued.
This Show Cause Hearing will take place on December 6, 2017 at 2:00 PM in the Public Services Conference Room on the
211 floor of City Hall, 211 S. Hamilton Street in High Point.

This serves as notification that MB Plating is in Significant Non -Compliance for the first half of
2017 for reporting violations, as defined in 15A NCAC 02H .0903(35)(F) and as adopted in the
City of High Point Sewer Use Ordinance Section 8-2-53(A):
Failure to provide, within 30 days after the due date, required reports such as baseline
monitoring reports, 90 -day compliance reports, periodic self-monitoring reports, and
reports on compliance with compliance schedules.
As of today, your January and February and March 2017 self-monitoring reports have not been
submitted. In February, the City was notified by Ms. Moore that production has decreased to
one or two days a week and requested reduced monitoring. The City concurred and reduced
the monitoring requirements.
Although we have been understanding of the situation your facility is experiencing, your facility
is not in compliance with its discharge permit; therefore, all overdue reports must be submitted
by June 15, 2017 to avoid escalating enforcement up to, and including water severance.
