RE: 84 Lumber —stormwater management application
Dear Ms McDaniel:
Please find enclosed (1) set of revised plans along with revised Stormwater Management
Statement and accompanying forms. The following are changes, additions, corrections per the
comments
1. Please complete and provide stormwater supplement form for each BMP. The
"Supplement EZ form" can be downloaded at the website
https://deg.ne.gov/about/divisions/energy-mineral-land-resources/eneW-mineral-
land- ermit- uidance/stormwater-bm -manual. RESPONSE: Form attached
2. Please complete and provide O&M form for each BMP. The "O&M EZ form"
can be downloaded from the same website link provided above. RESPONSE:
Form attached
3. Please fill out table IV. 10. (drainage area information) on the application form
SWU-101 (page 3) and provide engineering drawings that clearly identify and
delineate each drainage area to each SCM and proposed impervious surface
area(s) as listed under this table IV. 10. A breakdown calculation should also be
provided to support these numbers. RESPONSE: Form revised to include table
IV.10 Sheet C06 (Drainage Map) has been revised to indicate the drainage areas
to the sand filters and a summary table added to break down the impervious areas
to each sand filter.
4. Please provide soils report or other documentation which indicates/determines the
SHWT elevations for the proposed BMP location. RESPONSE: report attached.
Sheet C08 revised to add the water table on the cross sections

5. Please provide documentation from the NC Secretary of State or other official
documentation, which supports the titles and positions held by the signing official
listed m itemIII.T.­a o1- the application form i.e. MsTITe litany Cypher).
RESPONSE: Resolution attached
6. Please modify engineering drawings to include the entire footprint of each SCM
system with delineation of the maintenance accesses and easements. RESPONSE:
Sheet CO2 (Utility Plan) and C05 (stormwater plan) revised to show a 10' access
easement to each sand filter along with an easement around the filter.
7. Please modify plan sheet #C-08 to show elevation of the flow bypass devices.
RESPONSE: Sheet C08 revised to add elevations of by-pass devices to the cross
sections.
8. Please modify plan sheets #C-05 and C-06 to include designed dimensions and/or
surface area of each sand filter unit. RESPONSE: Sheets C05 and C06 revised to
add dimensions to the sand filters.
