The permit expiration date is February 28, 2018. As required, the Town is submitting a permit renewal application package. The renewal package includes one original and one copy of- - f- Cover letter - Completed Application Form CSA 04-16 - Pump station list - High priority lines list - Annual Budget for Collection System (current fiscal year) - Capital Improvement Plan - Response action plan - Contingency Plan - Sewer collection map. Comments related to the permit application include: • The current permit has eight simplex pump stations included as part of the Town's collection system. These pump stations are privately owned and are not owned or operated by the Town. We request that the eight simplex pump stations be removed from the permit. • Approximately 1.5 miles of new gravity lines have been added to the collection system since permit issuance and this addition is reflected in the permit renewal application. • The Town has a collection system map which shows the location of the sewer lines, manholes and pump stations. The as -built plans for the collection system are available and these plans show the depth and size of pipes. The collection system map was on the Town's GIS system. The server became corrupted and the data was lost. The Town has purchased GIS location equipment and mapping software and will be mapping the collection system per permit requirements. Failure to submit all required items will necessitate additional processing and review time. For more information, visit the System -verde Collection Sy stem Permitting website General — When submitting an application to the Pretreatment, Emergency Response, & Collection Systems (PERCS) Unit, please use the following instructions as a checklist in order to ensure all required items are submitted. Adherence to these instructions and checking the provided boxes will help produce a quicker review time and reduce the amount of requested additional information. The Applicant shall submit one original and one copy of the application and supporting documentation. A. Cover Letter ❑ Submit a cover letter listing all items and attachments included in the permit application package B. No Application Fee Required ➢ No application fee is necessary. The permittee will be billed an annual fee upon issuance of the permit ➢ The appropriate annual fee for systemwide wastewater collection system permits may be found at ➢ Annual Non -Discharge Fees C. System -Wide Wastewater Collection System (FORM: CSA 04-16) Application: ❑ Submit the completed and appropriately executed System -wide Wastewater Collection System (FORM CSA 04- 16) application. Any unauthorized content changes to this form shall result in the application package being returned. If necessary for clarity or due to space restrictions, attachments to the application may be made, as long as the attachments are numbered to correspond to the section and item to which they refer. ❑ If the Applicant Type in Section I3 is a Privately -Owned Public Utility, provide the Certificate of Public Convenience and Necessity (CPCN) from the North Carolina Utilities Commission demonstrating the Applicant is authorized to hold the utility franchise for the area to be served by the wastewater collection system, or ❑ Provide a letter from the North Carolina Utilities Commission's Water and Sewer Division Public Staff stating an application for a franchise has been received and that the service area is contiguous to an existing franchised area or that franchise approval is expected. ❑ If the Applicant Type in Section I.3 is a corporation or company, provide documentation if it is registered for business with the North Carolina Secretary of State D. WQCS00177 Attachment A for Condition V(4) There are only four High Priority sewer collection lines in Newport. They are as follows: 1, Newport River Crossing US 70. This is a directional bore under the river and adjacent wetlands. The ends of the lines are readily visible from the highway. 2. Newport River Crossing US 70 Business This is a directional bore under the river and adjacent wetlands. The ends of the lines are on the shoulder of the road and are easily accessable. 3. Creek crossing at WWTP. This line is visible from the plant and is visibly checked daily. 4. Westfield Rd. ditch'crossing. This line is also visible from the plant entrance road, and is visibly checked daily. Upon receipt of call, staff will dispatch utility crew. Responding Staff 1. Working Hours Staff responding immediately to the incident shall promptly investigate the cause and stop the sanitary sewer overflow. Should make every attempt to protect storm drain inlets and entrances of waterways from overflow material. If the problem appears to be a blockage The jet rodding machine is used to clear the blockage. Investigate the blockage to determine what preventative measures may be required to prevent further blockage of this type or at this location. TV inspect for roots, line breaks, etc. If the problem stems from a lift station failure: Staff troubleshoots the lift station and makes necessary repairs to stop the Sanitary Sewer Overflow. Arrange for Stand by Emergency pumping as may be needed. If the problem stems from another type of failure: Evaluate the type of failure, such as a line collapse or breakage. Arrange for pumps, etc. for a temporary bypass to the downstream manhole. Excavate the problem using all necessary safety precautions that the situation may dictate. Make the needed repairs. Cleanup and notification procedures. - The site secured and cleaned up as soon as the repairs are made. Solids are removed and all of the affected area is thoroughly limed with quicklime (burnt lime or calcium oxide) 1 Sanitary Sewer Overflow Response Plan Town of Newport or sprayed with 1 % bleach solution as applicable. Whenever possible, the spilled wastewater is contained and pumped back into the collection system Notify NCDWQ Wilmington (1-910-395-3900 or 1-800- 858-0368) about the spill, its causes, and the response actions taken. Complete a spill report for the files, and forward to the Wilmington Office as required (fax 1-910-350- 2004) 2. The supervisor or designee will notify any other employees needed and notify any outside agencies, as required. 3. After Hours on Call Person, upon being dispatched to a sanitary sewer overflow, will respond within 45 mins. Should make every attempt to protect storm drain inlets and entrances of waterways from overflow material. The on call person will contact supervisor for help. The investigation will be done the next working day. Afterhours cleanup procedures: The site secured and cleaned up as soon as the repairs are made. Solids are removed and all of the affected area is thoroughly limed with quicklime (burnt lime or calcium oxide) or sprayed with 1 % bleach solution as applicable. Whenever possible, the spilled wastewater is contained and pumped back into the collection system. The supervisor will notify NCDWQ. 2 Sanitary Sewer overflow Response Plan Town of Newport Access to the sewer cleaning equipment is at the town waste water plant. All employees have keys to the equipment. The lime and bleach is stored at the towns waste water plant. In the case of pump station failure, the following steps will be taken: • Type of failure will be determined Mechanical or Electrical • Electrical Failure ➢ Pumps station #7 has a hookup for a portable generator ➢ Spare parts — fuses, breakers, alternators, relays, etc — are available for the more common failures. ➢ Electrical contractor will be called as needed. • Mechanical Failure ➢ Pump will be inspected to check for blockages ➢ Control floats will be checked ➢ Belts will be checked on belt -driven pumps ➢ Bypass pumping connections are installed at all pump stations. The Town has four portable pumps • Pump Failure ➢ If pump cannot be made operational on-site: ■ Pumps will be sent to pump/motor repair shop or pump repair company will be contacted for on-site repair. • If both pumps in the pump station fail or if the remaining pump cannot maintain the water level: ➢ The Town's vacuum truck will be utilized to pump the water level down. ➢ A septage hauling company will be contacted to pump out the pump station wet well as needed. ➢ Bypass pumps are available as needed.
