I have attached the certificate of coverage for NCS000136. I will mail out the original signed today. Unfortunately, our
computer system will not allow me to capitalize the ‘x’ in SpecGX. It is listed correctly in the COC but please let me know
if it does not look correct.

Hi Bethany! Mallinckrodt submitted a permit mod request to change our ownership from Mallinckrodt LLC to SpecGx
LLC. We received a response from NCDEMLR indicating the permit was not being reissued but simply a new cover page

being provided. I did not see a new Stormwater Permit Cover Page as noted in the cover letter. What was attached
appeared to be a Permits Contact summary Form for our facility from a NCDEMLR database. I scanned the 2 documents
that were in the envelope. Please confirm whether or not the Permit Cover Page was in fact missing.

I also noticed on the Form that SpecGx LLC has a lower-case “x” rather than a capital “X.” This should be corrected in
the database and the permit cover page, if it appears there the same way too.
Thanks in advance for your help with this!
