This report is to document sediment loss at Site 24 and resulting discussions and actions..
Sediment lost into wetland area between Basins ID 27.14 and 27.15.
-Temporary diversions were installed prior to rain event but it proved to be too low and not well enough
compacted to prevent failure. It was also poorly placed where runoff couild not easily get into the slope drain
to the basin ID 27.15. Storm water runoff broke thru the temp diversion and bypassed the basin.
-Silt fence along the toe of the slope captured some but sediment overtopped it and settled in the wetland area.
No sediment loss was seen thru the pipe under the highway from this site.
-NCDOT and contractor inspectors found the loss during NPDES inspections and directed Dragados personnel
to make necessary repairs and upgrades to the temp diversions in this area.
-Contractor responded and began corrective action and as soon as the site was reviewed initiated clean up per
the instructions of the DEO.
-Discussions were held with Dragados management about specific repairs and additional actions - these
included good compaction on the temp diversion berms, ensuring slope drains are opened up and not clogged
with soil or with stone meant as inlet protection. Also discussed was directing storm water to the existing
basins and utilizing properly installed diversion berms and slope drains at all locations.
-Additionally, another temp diversion was used to divide the drainage area approaching the basin ID 27.15 and
the contractor was directed to carry the remaining storm water flow across the site to basin ID 27.14 with a well
compacted temp berm along the road edge at the top of the slope.
All corrective work was underway at this time.
-Additional discussion centered around making sure all employees understand the policy agreed to on stopping
daily operations to focus on EC needs and the need for attention to detail on installation of EC devices. Follow
up checking is needed by responsible Level II supervisors for that section at all permitted sites prior to
predicted rain events. Both basins are well constructed and maintained, but are only effective if the storm
water can get into the basins.
