Environmental Management Commission implementing that Article, shall be guilty of a misdemeanor punishable

Dear Mr. Roberts:
Division personnel have reviewed and approved your application for renewal of the subject permit.
Accordingly, we are forwarding the attached NPDES discharge permit. This permit is issued pursuant
to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement
between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as
subsequently amended).
This final permit contains no significant changes to its terms from those found in the draft permit sent to
you on April 9, 2014. However, you are reminded of the new condition requiring electronic reporting of
discharge monitoring report (DMR) data using the Division's eDMR internet application. This new
requirement will become effective on June 1, 2015 (approximately 270 days or nine months following
the effective date of the permit).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty
(30) days following receipt of this letter. This request must be in the form of a written petition,
conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of
Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless
such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may
require modification or revocation and reissuance of the permit. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Resources or
permits required by the Division of Land Resources, the Coastal Area Management Act or any other
Federal or Local governmental permit that may be required.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revolted. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.
Aqua North Carolina, Inc.
is hereby authorized to:
1. To discharge filter backwash wastewater from an AdEdge water treatment facility that includes the
following components:
• AdEdge AD26 (manganese dioxide) filters in parallel
• Two 5,000 gallon bacicwash/sludge storage tanks
• One additional sludge tank
• Recycle pump
• Sedimentation filter
The facility is located in Holly Springs at the Hopson Downs Subdivision WTP Well #4, at Spence
Farm Road in Wake County.
2. Discharge from said treatment works at the location specified on the attached map into an unnamed
tributary to Basal Creek, classified B -NSW waters in the Neuse River Basin.
3. This facility shall be operated as a 100% recycle system. Discharge to surface waters shall occur
only during extraordinary circumstances and shall meet all effluent limitations found in Part A. (1.)
of this permit on those occasions.

discharge monitoring reports electronically using NC DWR's eDMR application system. See
Condition A. (2.).
2. For instantaneous flow monitoring, the duration of the discharge must be reported in addition to the total
flow. All volumes discharged are to be reported on discharge monitoring reports (DMRs)
3. Additional monitoring is not required after two (2) discharge episodes have already been sampled during
the reporting month
4. The facility shall report all effluent TRC values reported by a NC certified laboratory, including field
certified However, effluent values below 50 µg/L will be treated as zero for compliance purposes.
5. TN = TKN + NO3-N + NO2-N, where TN is Total Nitrogen, TKN is Total Kjeldahl Nitrogen, and NO3-N
+ NO2-N are Nitrate and Nitrite Nitrogen, respectively.
All samples must be taken during a representative discharge event.
There shall be no discharge of floating solids or visible foam in other than trace amounts.
There shall be no discharge of waste or chemicals that do not directly result from the responsible treatment of
raw water.
The discharge ditch shall be lined or have rip rap to prevent erosion and washing of soil and silt to the creek.

SLUDGE MANAGEMENT PLAN
For
Aqua North Carolina, Inc.
No sludge will be treated on any wastewater treatment plant site operated
by Aqua N.C., Central'Divi.sion.
Where practical, sludge removed from a Aqua N.C., Central Division
facility will be transported via a contract hauler to another Aqua N.C.,
Central Division facility for the purpose of "seeding" a new or under loaded
plant.
Unusable (or "dead") sludge will be removed by a contract hauler and
properly disposed of in accordance with NCGS 14s-215.1. Contract
haulers used by Aqua N.C., Central Division will be required to report the
quantity of sludge transported and identify the location of the proposed
disposal site if the sludge is not taken to an existing plant operated by Aqua
N.C., Central Division, Aqua N.C., Central Division has not entered into
any agreement to accept sludge into its facilities from plants not owned by
them.
Aqua N.C., Central Division will keep records on the quantity of sludge
removed from each facility, the name of the contract hauler, and the
destination of the sludge (whether used in another plant or disposed of).
The information will be kept on file and will be made available to any
regulatory agency having jurisdiction over sludge treatment or disposal.
Aqua N.C., Central Division includes facilities ander the jurisdiction of the
Raleigh, Wilmington and Fayetteville Regional Offices.
