REQUEST FOR MORE INFORMATION
Dear Sir or Madam:
On November 25, 2008, the Division of Water Quality (DWQ) received your After-The-Fact application
dated May 20, 2008, to fill or otherwise impact 0.222 acres of 404/wetland, 107 linear feet of perennial
stream impact and 5,750 square feet of Zone 1 Neuse River basin protected riparian buffer, and 2,446
square feet of Zone 2 Neuse River basin protected riparian buffer to construct the residential development
at the site. The DWQ has determined that your application was incomplete and/or provided inaccurate
information as discussed below. The DWQ will require additional information in order to process your
application to impact protected wetlands and/or streams on the subject property. Therefore, unless we
receive five copies of the additional information requested below, we will place this project on hold as
incomplete until we receive this additional information. If we do not receive the requested information,
your project will be formally returned as incomplete. Please provide the following information so that we
may continue to review your project.
Additional Information Requested:
1. Please re-submit your site plans on full plan sheets at a scale of no smaller than 1 "=50' with
topographic contours shown.
2. Please provide cross section details showing the provisions for aquatic life passage.
3. Please provide building envelopes for all lots with wetlands, streams or buffers on the site plans.
4. Please locate any planned septic tanks and repair areas on the site plan.
5. Please enumerate all riparian buffer impacts on the site plan and clearly label impacts (Buffer
Impact 1, etc.).
6. Please locate all of the protected riparian buffers as overlays on the site plan, clearly showing
Zone 1 and Zone 2.

7. Please provide a qualitative indirect and cumulative impact analysis for the project. Please see DWQ's
policy for guidance on our website at:

With the exception of Scour Hole Number 1, all proposed scour holes are placed adjacent to filter
strips that exceed the maximum allowable slopes for these devices. Please redesign with
appropriate devices to meet the diffuse flow requirements per Chapter 8 of the BMP Manual.
Please provide a BMP Supplement Form for each proposed best management practice (including
both worksheets, Design Summary and Required Items Checklist), as well as all of the required
items. The BMP Supplement Forms are available on the DWQ web site at:

9. Level Spreader Number 1 is placed adjacent to a filter strip that exceeds the maximum allowable
slopes. Please provide an alternative method for achieving diffuse flow per Chapter 8 of the
BMP Manual. Please provide a BMP Supplement Form for any proposed best management
practice that will be used at this location (including both worksheets, Design Summary and
Required Items Checklist), as well as all of the required items. The BMP Supplement Forms are
available on the DWQ web site at: http://h2o.enr.state.nc.us/su/bmp_forms.htm.
10. Level Spreaders Number 2 and 3 are directing stormwater toward the road fill slope. Please
relocate these devices such that the filter strip is completely outside the road fill area.
Please submit this information within 30 calendar days of the date of this letter. If we do not receive this
requested information within 30 calendar days of the date of this letter, your project will be withdrawn
and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
