Dear Mr. Brinn.
A letter of request for an Authorization to Construct was received June 1, 1989 by the
Division and final plans and specifications for the subject project have been ieviewed and found to
be satisfactory. Authorization is hereby granted for the construction of a 0.325 MGD wastewater
treatment facility consisting of a 0 406 MG aeration tank, a 0.096 MG clarifier, a 36,500 gallon
sludge holding tank, tertiary filters with approximately 208 square feet of surface area, gaseous
chlorination, a 9,375 gallon chlorine contact tank, and continuous flow measuring device, and
modification to the existing facilities to include an aerated equalization basin and mudwell for
tertiary filters, to serve Carolina Trace Subdivision with discharge of treated wastewater into Upper
Little River.
This Authorization to Construct is issued in accordance with Part III, paragraph B of the
NPDES Permit No NCO038831 issued March 31, 1989, and shall be subject to revocation unless
the wastewater treatment facilities are constructed in accordance with the conditions and limitations
specified in Permit No NC0038831.
Upon 50% completion of the subject facilities, the Permittee must employ a certified
wastewater operator in accordance with Part III paragraph C of the referenced permit
The sludge generated from these treatment facilities must be disposed of in accordance with
G.S. 143-215.1 and in a manner approvable by the North Carolina Division of Environmental
Management. Carolina Trace Corporation shall submit a sludge disposal plan and received
approval prior to operation of the subject facilities within 180 days after issuance of this
Authorization to Construct.

The Raleigh Regional Office, telephone number 919/733-2314 shall be notified at least
twenty-four (24) hours in advance of operation of the installed facilities, so that an in-place
inspection can be made. Such notification to the Regional Supervisor shall be made during the
normal office hour from 8:00 a.m. until 5.00 p.m. on Monday through Friday, excluding State
holidays.
In event the facilities fail to perform satisfactorily in meeting its NPDES permit effluent
limits, the permittee shall take such immediate corrective action as may be required by this
Division, including the construction of additional wastewater treatment and disposal facilities.
The sand media of the tertiary filters must comply with the Division's sand specifications.
Upon completion of construction and prior to operation of this permitted facility, a
certification must be received from a professional engineer certifying that the permitted facility has
been installed in accordance with the requirements of the NPDES Permit and the approved plans
and specifications. Mail the Certification to the Permits and Engineering Unit, P.O. Box 27687,
Raleigh, NC 27611-7687.
A copy of the approved plans and specifications shall be maintained on file by the
Permittee for the life of the permit.
