Dear Sue:
On behalf of the Village of Clemmons, we are requesting a permit modification to the 401 Water Quality Certification for
the above-mentioned project. We are requesting a modification to a condition in the Water Quality Certification #3886
document, specifically condition number 13, "Placement of Culverts and Other Structures in Waters and Wetlands".
Our request is the removal of the requirement to depress the culvert invert below the stream bed for the 66 -inch diameter
culvert as shown on Exhibit C dated 11-17-15 and revised on 1-14-16, attached for reference. The upstream end of the
culvert isn't located in a stream and connects to an existing closed storm drainage system.
It should be noted that this condition existed during the permitting stage of the project and a request to exempt the above-
mentioned culvert from that specific requirement should have been made.
