JAY ZIMMERMAN Dtffldor RECEIVEDINCDEQ/M AUG -12017 Water Quarry Permitting Section On July 12, 2017, Paul DiMatteo of the Division of Water Resources ("Division"), Winston-Salem Regional Office conducted a Compliance Evaluation Inspection of the above referenced wastewater treatment system. Dana Newcomb, Operator in Responsible Charge (ORC), was present for the inspection. An inspection summary follows and an inspection report is attached for your records. Site Review 1. Permit The current permit became effective on August 1, 2012 and expired on April 30, 2017. A new permit has been drafted; the facility is permitted to operate under the expired permit until a decision is made on the new application. A copy of the current permit was available for review during the inspection. Permitted components include an oil -water separator and polishing filters. 2. Facility Site Review The facility was as described in the permit. All water that may potentially be contaminated with diesel fuel or oil is sent to the oil -water separator and filters before being discharged. 3. Operation and Maintenance All components appeared to be well maintained and functioning properly. Ms. Newcomb or a properly designated backup ORC visit the facility as required. 4. Flow Measurement Flow is measured using pump log data. Effluent/Receiving Waters Effluent is discharged into an unnamed tributary to Little Jacob's Creek. Access to the outfall is well maintained. Water discharging through the outfall during the inspection appeared to be clear with no visible impact to the receiving stream. Records/Reports Discharge Monitoring Reports (DMRs) and lab data are maintained according to the permit. DMRs include all permit parameters. Copies of DMRs, operator visitation logs, lab reports and Chain -of -Custody forms were available for review during the inspection. Self -Monitoring Program A review of DMRs from April, 2015 through May, 2017 showed no violations. A comparison of submitted DMR data and lab records showed no errors or concerns. Laboratory All required parameters are measured by Research and Analytical Laboratories under certification # 34. All samples are taken from the outfall. Chain -of -custody forms were complete and available for all parameters. We appreciate your efforts in effectively operating and maintaining this treatment system.
