REQUEST FOR MORE INFORMATION
On October 6, 2008, the Express Review Program of the Division of Water Quality (DWQ) received
additional information from Ms. Robin Eddy regarding the above referenced project. The DWQ has
determined that your application still requires additional information. Please provide the following
information so that we may continue to review your project and prevent return of your application as
required by 15A NCAC 2H .0506:
Additional Information Requested
Diffuse Flow Provision Measures:
a. Drainage Area 2
i. Please submit the Level Spreader Supplement for Drainage Area 2 with all items
listed in the required items checklist.
ii. For the level spreader for Drainage Area 2, please show a profile view or cross
sectional details on the plan sheet, including the upslope channel, level spreader lip,
and downslope filter fabric underneath a 3-foot #57 rip rap apron. (See Fig 8.2 of the
NC DWQ BMP Manual for more details).
iii. Please avoid using masonry blocks to construct the level spreader lip. The lip (stem)
shall be constructed as a "pour in place" concrete structure.
iv. Since there is no bypass channel for larger storms, the drawdown from the BMP shall
be that resulting from routing the 10-year storm through the BMP. Please provide
routing calculations to support any stated drawdown.
v. Please include a signed and notarized Operation and Maintenance Agreement for the
proposed level spreader.
b. Drainage Area 3
i. Down slope of Level Spreader for Drainage Area 3 is too steep (9.3%). The photo
included in your submittal does not indicate the presence of grass or dense vegetation
cover as indicated on the Level Spreader Supplement Form. Combining these factors
with the proposed dry detention storage will not achieve the necessary nutrient
removal required for a project located in an NSW. For Drainage Area 3, please
submit a properly designed BMP such as a bioretention with supporting information
to meet at least 85% TSS and 30% TN removal efficiencies.

2. Minor Variance:
a. Please provide a detailed plan map of specifically how the all impervious surfaces on the lot
will be collected and treated (e.g. show roof gutter system to stormwater collection system,
etc.).
b. Your proposed onsite mitigation is unacceptable. Please either provide an appropriate
restoration site and plan or provide an NCEEP acceptance letter noting their willingness to
provide 2,850 f 2 of riparian buffer mitigation credits in the corresponding HUC.
c. Since you have not provided a detailed construction plan of Lot #8 it is impossible to
determine potential impacts. Please provide a house footprint and a building envelope.
Please prove that there will be no temporary impacts to Lot #8 in your detailed construction
plan and clearly enumerate the impacts on an impact map. If the project will result in
temporary impacts, then please provide a written description of temporary impacts to Zone 2
of the riparian buffer (how the building envelope which includes the area surrounding the
house disturbed during construction) will be limited to Zone 2 and what method will be used
to restore the area (aerating and planting native grass, etc)).
d. Please include minor variance request Zone 2 impacts for Lot #8 on your PCN form and
complete Section X.3 regarding buffer mitigation.
Please Note: This project will be returned if the items listed in the first two requests for more information
letters are not adequately addressed.
Please respond within five (5) days of the date of this letter by sending two (2) copies of the above
information in writing. The Express Review Program is a process that requires all parties to participate in a
timely manner.
This letter only addresses the application review and does not authorize any impacts to wetlands, waters
or protected buffers. Please be aware that any impacts requested within your application are not
authorized (at this time) by the DWQ.
