Sullins, Director Division of Water Quality October 2, 2008 DWQ Project # 06-1955, Ver. 3 Johnston County CERTIFIED MAIL: RETURN RECEIPT REQUESTED Mr. Michael Coates Crossroads Development Corp 12400 NC 50 North Willow Springs, NC 27592 Subject Property: Greystone Crossing Phase 3 Hooks Branch [030404, 27-45-4, C, NSW] REQUEST FOR MORE INFORMATION Dear Mr. Coates: On July 21, 2008, the Division of Water Quality (DWQ) received your application dated June 23, 2008, to fill or otherwise impact 0.091 acres of 404/wetland, and 7,348 square feet of Zone 1 Neuse River basin protected riparian buffers and 4,120 square feet of Zone 2 Neuse River basin protected riparian buffers, to construct the proposed residential development at the site. On September 5, 2008, the DWQ received additional information from you, however, the DWQ has determined that your application remains incomplete and/or provided inaccurate information as discussed below. The DWQ will require additional information in order to process your application to impact protected wetlands and/or streams on the subject property. Therefore, unless we receive five copies of the additional information requested below, we will place this project on hold as incomplete until we receive this additional information. If we do not receive the requested information, your project will be formally returned as incomplete. Please provide the following information so that we may continue to review your project. Additional Information Requested: 1. Please provide the current version of the Required Items Checklist, available at: http://h2o.enr.state.nc.us/suibmp forms.htm, and ALL required items were not included in your recent submittals at the appropriate scales. Failure to provide this information in your next submittal will result in a return of application. 2. Please plan details for all scour holes at a 1" = 20' scale or larger. 3. Please provide a current version of the Level Spreader Operation and Maintenance Agreement for all level spreaders and scour holes, available at: http://h2o.enr.state.nc.us/su/bmp forms.htm. 5. Please address the following issues regarding specific discharge points: a. The discharge points between lots 56 and 57 and lots 162 and 163 must be equipped with appropriate diffuse flow devices per Chapter 8 of the BMP Manual. If you propose to use level spreaders, then you must provide one Level Spreader Supplement Form for each proposed level spreader, available at: http://h2o.enr.state.nc.us/su/bmp forms.htm. Be sure to include the Required Items checklist and all items listed therein. b. The discharge point between lots 182 and 183 must be equipped with a flow dissipater device that discharges to the wetland at a non-erosive velocity of two feet per second during the ten-year storm event. Please submit this information within 30 calendar days of the date of this letter. If we do not receive this requested information within 30 calendar days of the date of this letter, your project will be withdrawn and you will need to reapply with a new application and a new fee. This letter only addresses the application review and does not authorize any impacts to wetlands, waters or protected buffers. Please be aware that any impacts requested within your application are not authorized (at this time) by the DWQ.
