Responsible party: PSNC Representative — Ben Childers Telephone (704) 718-4234 Contractor performing HDD: Lone Star Directional Drilling Date and time of IR: Saturday, June 3, 2017 at 12:45 pm. How was IR detected: Colter Franks (Lone Star Directional Drilling - Foreman) noticed drilling mud not returning to the mud pit and walked down to the river and saw IR on far bank of river. Location of the IR: Broad River at Station 1880+00 approximately 25' right of ROW, downstream of large mid -stream sandbar. See attached map. Type of material: Bentonite clay and water slurry — standard HDD drilling mud. Description of IR and efforts to protect sensitive areas: The drilling operation was stopped immediately upon noticing cloudy water along the west bank of the river. The source of the drilling mud release was identified to be at the toe of the slope on the west bank. Tom Perez (S&ME Environmental Inspector) was contacted following cessation of drilling operations. No drilling was conducted on Sunday, June 4, 2017. On Monday, June 5, 2017, the contractor infused Polly Swell product into the drilling mud mixture to inhibit further release of drilling mud through the fracture. The Poly Swell material required at least 12 hours to "set up," during which time no additional drilling operations were conducted. Joey Lawler (S&ME Project Manager) reported incident to Amy Annino of the North Carolina Department of Water Resources (Asheville Region) by phone and David Brown of the US Army Corps of Engineers (USACE) on Monday, June 5, 2017. Drilling operations remained idle on Monday, June 5, 2017. On Tuesday, June 6, 2017, Latex Construction (General Contractor) installed a turbidity curtain on the west bank of the Broad River at the IR location and three additional turbidity curtains further downstream to contain any further drilling mud that may flow into the river when drilling • ' operations resume. Again, no drilling was performed on Tuesday, June 6, 2017. Tom Perez performed an inspection June 6, 2017, assessing the containment measures and documenting conditions. Estimated size of IR (quantity and area): A fine layer of drilling mud impacted the river bed for approximately 150 feet. Due to the quick action of the contractor and environmental inspector, a minimal amount of drilling mud entered the river prior to the halt in drilling operation. The estimated impact was approximately 500 gallons. Proposed additional HDD activities: The designed length of the HDD is approximately 2,000 feet. Inadvertent returns were noted during the drilling of the initial pilot hole. A combination of the Poly Swell and impermeable turbidity curtain containment should minimize further release when drilling operations resume. There will be 3 reams (18" reamer, 28" reamer, and 36" reamer) after the pilot hole is completed before pipe pull back, and 2 to 3 weeks before the entire HDD is complete. Proposed final cleanup method: On completion of HDD activities, Lone Star Directional Drilling will remove discernible drilling mud or turbid water from within the containment area. If necessary, the mud will be removed by hand or vacuum truck as appropriate. Upon completion of clean-up activities, turbidity curtains and other containment materials will also be removed. Party responsible for cleanup: Lone Star Directional Drilling General comments: It appears that a majority of the drilling mud will be contained in the turbidity curtains so as to minimize the impact to the river. The contractor immediately ceased drilling operations, and acted in good faith to contain the drilling mud, and immediately notified environmental monitors. The contractor and inspectors will continue to monitor the area, remain alert for rain events and take actions necessary to contain drill mud/turbid water in impacted area. Drilling mud was coming through a fracture at the toe of the west bank of the Broad River at approximately Sta 1880+00 (25' north of the easement). The HDD is generally perpendicular to the river. Drilling was stopped immediately upon discovery.
