The North Carolina Department of Transportation is scheduled to replace Bridge #149
wiih a 3-sided bottonaless culvert at the same locatian. The existing single span timber
and steel bridge is 25' long by 20' wide. The new bottomiess c�lvert will be 24' wide by
3' tall by 61.5' long. Class II riprap will be countersun� inside c�f the structure to
maintain channel width. No on-site detour is proposed.

walls. It includes the floodplain construction as well. Tmpacts are higher than nor�nal due
to the skevv of the crossing.
The repiacement of the e�sting bridge with the proposed ne�vv bot�aml�ss culvert will
require temporary dewatering duxirig footer constructian and riprap installation. An
imperviotzs �andbag dike will be constnzcted upstreatn of the proposed impact area to
create a small pool. Clean water will temporarily be pumped around or piped thraugh the
constructian area and will be discharged downstream. Any dirty vvater lacated inside the
constructian area �will be pumped inta a silt bag i� necessary. Once the new structure is in
pla.ce, the impervious dikes will be removed and preconstruction streambed elevations
will be �estored.
Construction of th�is project will require impacts to waters of the United States in the New
River Basin. The existing bridge crosses Grassy Creek {Class: WS-IV -}-} arid is �ocated
in a trout watershed. This project dves not drain to designated critical habitat fvr the elk
toe mussel.
This project vvas reviewed for the presence of threatened and endangered species in the
field and I have also included a query of the NCI�JHP database. Based on the project area
mapped with my reques°t, the database seareh indicates that there are no records of any
federally listed threatened or endangered species, impartant natuaral cozn.munities, natural
areas, or cozzsexva#ion/managed areas within the propased project boundary. There will
be ap�roximately 0.2 acres of tree cutting at the project lc�catian due to cut and fill work.
The NLEB is listed as a protected species in this county but the project daes not fall
within 0.25 miies af any red HCJC identified in the SLOPES spelled out in the
PJatic�nwide Permit Regional Conditions. We do not anticipate any irnpact to any other
federally listed species.
Cultural resource compliance woxk has beezx cazx�pleted. No archaeological resaurces ar
historic properti�s will be affected by this praject (see attached).
The project impacts are not significant in nature and shauld nat remove or degrade
existing water quality uses. Stoa�mwater will he routed through girass vr rack lined
ditches, depending on the ditch line slope. Pipe sizes �xsed will be the minimutn
necessary to create a safe roadway. By utalzzing the exosion eantrol measures outlined in
the attachments ta this applicatian, downstream water quality standards will be protected.
,All appropriate BMPs will be used during construction.
Attached is a Preconstruction Notification Form, plan sheets with proposed erosion
control measures, a marked county map and a mar�ed topography map.
