MEMORANDUM
To Heath Slaughter, Division Environmental Officer, NCDOT Division 11
From Amy Euliss
Subject Site Visit Follow-up
TIP No K-4703
Wilkes County
This memo is a follow-up to our site visit and Construction Stormwater Inspection of the Rest Area in
Wilkes County (TIP No K-4703) on August 13, 2008 I appreciate the time you, David Poindexter, and
other DOT and Contractor staff spent with George Smith and myself discussing the Construction
Stormwater records and reviewing permitted sites
The following items are provided to help document our site visit and to assist the Division NCDOT staff
with current project status documentation We visited all permit sites on this date, which will be
discussed below
Construction Stormwater Records Records kept by both NCDOT and the contractor fully complied with
the applicable requirements of the NCG01000 The Division appreciates NCDOT's immediate attention
to deficiencies in the records noticed during a site inspection on July 24, 2008
Site 1 The cause of the sedimentation problem has been attributed to damage to the pipe during a water
line installation The issue has been addressed Please continue to monitor this site, and remove any
sediment that accumulates in the in-stream check dam until the pipe has completely flushed any
accumulated sediment Once the pipe has stabilized, the check dam should be removed
Site 2 An in-stream check dam that was installed with the pipe extension needs to be removed
Sites 3 The site has been constructed according to plans The constructed riffle has not begun to silt in
We'll need to continue to monitor its success
Site 4 The impacts to this site extend beyond the 59 feet of permanent impacts I will need to perform a
permit modification for the additional impacts Please send me a copy of the modified plans as soon as
possible
Thank you again for the time you took to conduct the site visits Please contact me if you have any
additional questions
