A Compliance Evaluation Inspection was performed on the subject facility by Ms. April Norton
on March 23, 2017 at 10:00 am. Mr. Michael Swan, Operator in Responsible Charge (ORC), and
Mr. Tim Fitzgerald were present for the inspection. The inspection is comprised of an overall
evaluation to document compliance and noncompliance of. facilities equipment, capability to meet
effluent limits, and terms & conditions with the National Pollutant Discharge Elimination System.
(NPDES) permit.
Permit
The NPDES permit became effective August 1, 2009 and expired on April 30, 2014.
An Authorization to Construct (ATC) was issued on October 9, 2013. Authorization is granted for
construction of modifications to the existing 6.2 million gallons per day (MGD). The authorization
results in an increase in design capacity to 10 MGD, but the limits permitted capacity to 8.2 MGD.
The following specific modifications have been granted: New Biological Nutrient Removal (BNR)
system consisting of four parallel trains with total volumes of 0.50 MG anaerobic zone, 0.91 MG
first stage anoxic zone, 4.95 MG aerobic zone, 1.48 MG second stage anoxic zone, 0.29 MG re=
aeration zone, including fine bubble diffusers and 40 vertical mixers; new blower/NRCY building
including two 6,074 inlet cubic feet per minute (ICFM) blowers, two 4,050 ICFM blowers, and
four 6,944 gallons per minute (GPM) NRCY pumps; a new clarifier; two 3,550 GPM return
activated sludge (RAS) pumps; a new process drain station including one 800 GPM pump, two
150 GPM non -potable water pumps; associated electrical and control/supervisory control and data
acquisition (SCADA) work, site work, and yard piping in conformity with project plans,
specifications, and other supporting data subsequently filed and approved by the Department of
Environmental Quality.

The construction is taking place in three phases: Phase one consisted of constructing a new
preliminary treatment facility and odor control system; phase two included the addition of new
sand filter construction, a new building and effluent pump, and new earthen berms for flood
control. The final phase, phase three; involves replacing the current activated sludge system with
a five phase BNR system. The estimated completion date for construction is October 2017.
Instream monitoring requirements are waived if this facility is a member of the Yadkin -Pee Dee
River Basin Association. If the facility ends. its membership in the Yadkin -Pee Dee River
Association, instream monitoring requirements specified in the permit take effect.
Whole effluent toxicity tests are conducted quarterly using Ceriodaphnia dubia. The Ceriodaphnia
dubia test is assessed as a pass or fail test, and the November 2016 test assessed was reported as
failing. This resulted in the issuance of a notice of .violation (NOV) by the Aquatic Toxicity
' Branch..
Record's & Reports
Process controls and observations are recorded daily by shift personnel on log sheets. A log book
is, also available documenting the ORCs site visitation.
The Performance Annual Report (PAR) is current and available to the public.
All records must be kept for 3 years and sludge for 5 years. All laboratory records must be kept
for .5 years.
Facility Site Review
The facility is continuing to operate under the existing 6.2 MGD wastewater treatment plant flow
capacity consisting of influent screw pumps, mechanical bar screen, aerated grit chambers, influent
flow measurement, dual primary clarifiers, roughing filters, three aeration basins, dual secondary
clarifiers,, tertiary filters, alum phosphorus removal _facilities, dissolved air floatation (DAF), and
ultraviolet disinfection.
Effluent/Receiving Waters
The effluent to the Rich Fork Creek, which flows into the Yadkin -Pee Dee River Basin, Class C
waters.
Flow Measurement
The NPDES permit requires continuous flow recording and an annual calibration. The flow
measuring device was calibrated on October 19, 2016. The next annual calibration is due October
31, 2017.
Self -Monitoring Program
There were no.NPDES monthly monitoring violations. The only violation which occurred was due
to toxicity in November 2016.
Laboratory data verification was performed on April 2016 and November 2016 Discharge
Monitoring Reports (DMRs). There were no errors noted.
Laboratory
State certified laboratory services are provided in-house. The lab was not evaluated at the time of
inspection.
Operations & Maintenance
There appeared to be no O&M concerns at the time of the inspection. The facility operations and
maintenance superintendent is Mr. Rick Riley.
Sludge Handling/Disposal
Sludge records were not evaluated at the time of this inspection.
Compliance Status
The compliance evaluation inspection is satisfactory. All action items from the previous inspection
were completed and are acceptable. Thank you for your efforts.
