missing monthly Discharge Monitoring Reports(DMRs)for the subject wastewater treatment plant

Failure to submit a timely DMR subjects you to civil penalty assessments not to exceed$25,000 per
violation. If this facility is not or was not discharging,you are still required to submit a monthly DMR
indicating"no discharge"or"no flow."In accord with the NPDES Program,DMRs are due no later than
the last day of the month following the month of reporting, i.e., a June report is due no later than July 31
[Part II, Condition D(2); 15A NCAC 2B.506(a)].If you have records documenting that these reports were
submitted to the Division before the required deadline,please forward.
Please submit the above missing documents by April 27,2016.
