Per our conversation, I was placed as the responsible official and given signatory authority for
the Roaring Gap WWTP in January 2003 by our,then,general manager Jay Manner. At this time the
DMRs were being signed by Arther Hash. When he left the company in the summer of 2004 due to
injuries on the job I began signing all of the correspondences associated with the WWTP including
DMRs.
I have spoken to everyone on our current staff and no one here recalls Brad Perkins as being the
responsible official for the WWTP.
Hal Transou has been our ORC for approximately 18 years and is also granted signatory
authority for the WWTP.
