Dear Mr. Swinnie:
The Division has reviewed your application for the subject permit. To enable us to complete our review in
accordance with N.C.G.S. 143-215.1 and 15A NCAC 2H.0105, please provide additional or revised
information to address the following comments:
1. As per rule 15A NCAC 2H .0106,Filing Applications,(1);the application shall be signed as
follows:
in the case of corporations, by a principal executive officer of at least the level of vice' =president, or
his duly authorized representative, if such representative is responsible for the overall operation of
the facility from which the discharge described in the permit application form originates;
The application was signed by the ORC. Please provide documentation that the person that signed
the application meets the requirements in the rules and is duly authorized or resubmit the application
with the appropriate signature.
2. Form 1 Section XII - Please provide a description of the nature of the business, including all
processes that generate wastewaters such as weaving, dyeing, finishing and other operations
performed at the facility.
3. Form 2C, Part C—please prove at least one analysis for the pollutants listed in Part C. Testing for all
parameters is required for the textile mills category.
If no response is received within 60 calendar days [per 15A NCAC 2H.0107(b)],the project will be
deactivated and withdrawn from our review process and the documents recycled. If you have any questions,
