Dear Mr. Andrews:
Division personnel have reviewed and approved your application for renewal of the subject
permit. Accordingly, we are forwarding the attached final NPDES discharge permit. This permit is
issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the
Memorandum of Agreement between North Carolina and the U.S. Environmental Protection
Agency dated May 9, 1994 (or as subsequently amended).
If any parts, measurement frequencies or sampling requirements contained in this permit are
unacceptable to you, you have the right to an adjudicatory hearing upon written request within
thirty (30) days following receipt of this letter. This request must be in the form of a written
petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-
6714). Unless such demand is made, this permit shall be final and binding.
Please take notice that this permit is not transferable. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or
permits required by the Division of Land Resources, Coastal Area Management Act, or any other
Federal or Local governmental permits which may be required.

In compliance with the provisions of North Carolina General Statute 143-215.1, other lawful standards and
regulations promulgated and adopted by the North Carolina Environmental Management Commission, and the
Federal Water Pollution Control Act, as amended,
Ameri-Kart Corp.
is hereby authorized to discharge wastewater from a facility located at the
Ameri-Kart Corp.
Northeast Market Street Reidsville
Rockingham County
to receiving waters designated as an unnamed tributary to Lick Fork Creek in the Roanoke River Basin
in accordance with effluent limitations, monitoring requirements, and other conditions set forth in Parts
I, II, III and IV hereof.
This permit shall become effective July 1, 2007.
This permit and authorization to discharge shall expire at midnight on February 29, 2012.

All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked, and as of this issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under
the permit conditions, requirements, terms, and provisions included herein.
Ameri-Kart Corp. is hereby authorized to:
1. Continue to operate an existing 0.005 MGD package wastewater treatment facility with the
following components:

This facility is located off Northeast Market Street in Reidsville at the Ameri-Kart Corp. V;VVTP in

2. Continue to discharge non -contact cooling water from outfall 002 at the Ameri-Kart Corp. WWTP.
3. Discharge from said outfalls at the locations specified on the attached map into an unnamed
tributary to Lick Fork Creek, classified C waters in the Roanoke River Basin.

2. TRC limit will become effective December 1, 2008, while monitoring is required beginning July 1,
2007.
3. The effluent temperature shall not cause the temperature of the receiving stream to increase more
than 2.8°C. In no case shall the effluent temperature cause the receiving stream temperature to
exceed 32°C.
4. The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units.
THERE SHALL BE NO DISCHARGE OF FLOATING SOLIDS OR VISIBLE FOAM IN OTHER THAN TRACE AMOUNTS.
A. (3.) BIOCIDE CONDITION
There shall be no chromium, zinc or copper added to the treatment system except as pre -approved
additives to biocidal compounds.
The permittee shall obtain approval from the Division's Aquatic Toxicology Unit prior to the use of any biocide
(not previously approved by the Division) in the effluent discharged under this permit. Approval for use of any
biocide not previously approved should be requested at least 90 days in advance of any planned usage.

and 002 is the cooling water. DCR

I just did the draft renewal for this facility and we noticed that on the map they include with the
renewal application they note an OUTFALL 003.
According to our file they have an 001 and 002. That's the way I kept the draft permit that will go to
notice today.
In the meantime, would you mind checking with the facility on that and see whether something needs
to be done?

The Solids are removed from the waste water plant on the average of 3 times in a 12
month period of time.
Theo erator in charge, who is Meritech Inc, notifies Ameri-Kart when the plant should

be pumped.
Ameri-Kart contacts a license pumping contractor and they remove the solids.
The license contractor disposes it at the Eden N.0 City waste water plant

required to be maintained under Article 21 or regulations of the Environmental Management Commission
implementing that Article, or who falsifies, tampers with, or knowingly renders inaccurate any recording or
monitoring device or method required to be operated or maintained under Article 21 or regulations of the
Environmental Management Commission implementing that Article, shall be guilty of a misdemeanor punishable
by a fine not to exceed $25,000, or by imprisonment not to exceed six months, or by both. (18 U.S.C. Section
1001 provides a punishment by a fine of not more than $25,000 or imprisonment not more than 5 years, or both,
