A review of the October 2014 self-monitoring report for the subject facility revealed violations of the
following parameter.

10/31/14 Downstream Color None Monitor monthly
Remedial actions, if not already implemented, should be taken to correct the causes(s) of the noted
violation. Unresolved violations may lead to the issuance of a Notice of Violation and/or assessments
of civil penalties by the Division of Water Resources.
If the violations are due to a transcription error (data left off the DMR) please submit an amended
October 2014 DMR (two copies) to this office along with a cover letter asking for rescission of the
Notice of Deficiency. Please address your correspondence to Ms. Marcia Allocco at the letterhead
