Dave,
This project is in Guilford County on SR 3051 over an unnamed tributary of Little Alamance Creek. Knox Rd is right off of Exit 132 on I-85/40 adjacent to the Wendy’s Restaurant. There
was not a 401 permit but there
was a buffer authorization issued January 29, 2016 (see attached). I am also attaching a topo map which has the Lat/Long at the bottom. Let me know if you have any other questions.

Jerry,
I neglected to ask you for more info about the project. Please send more detail relative to the project for record keeping purposes. County, SR, permit number (if there was a 401written
permit) and lat
long. Thanks.

Dave,

Per our earlier conversation, attached are the pictures of the sediment that deposited in the stream beneath the Knox Road bridge. The sediment did not appear to wash down from the work
site because there was no
trace of sediment in front of the installed silt fence nor any evidence (ruts, displaced straw, matted vegetation) in front of the silt fence. Although we cannot be sure, it looked
as if the sediment was moved by a
groundwater discharge along the bank behind the fence. I instructed the contractor to remove the sediment (approx. 1.5 square yds) and monitor the site for any future discharges. Let
me know if you have any
questions or comments.
