Please provide a brief description of this project (attach site plan if available):
Project consists of a modification request of a previously approved temporary impact needed for a
sanitary sewer crossing. Further detailed design of this utility crossing in combination with the
existing elevations of a manhole on the adjacent property has determined the sanitary sewer casing
to be above the bed of the stream. Therefore this modification request is to propose a permanent
impact within the same footprint of the previously approved temporary impact of the sanitary sewer
crossing of Smith Creek. Additionally, impacts associated with stream stabilization are being
proposed.

N/A-Water Quality Certification 3699 does not include a condition that reauires stormwater
management, however the project does require diffuse flow. The previous approval of this project
included an approved diffuse flow and stormwater management plan.
