REQUEST FOR MORE INFORMATION-EXPRESS REVIEJV PROGRAM
Dear Mr. Skinner:
On September 12, 2005, the Division of Water Quality (DWQ) received your application for a 401 Water
Quality Certification and Riparian Buffer Authorization through the Express Review Program. The DWQ
has determined that your application package is incomplete; the DWQ will require additional information
in order to process your application to impact protected wetland, streams and/or buffers on the subject
property. Unless we receive the additional information requested below, we will have to move toward
denial of your application as required by 15A NCAC 2H .0506 and will place this project on hold as
incomplete.
Please provide the following information so that we may continue to review your project:
Your acceptance letter into the Express Review Program specified that you must send in a complete
application package including the Addendum. You have failed to provide a complete package in
accordance with the instructions. Please provide a complete PCN application and supporting plans,
calculations, worksheets, calculations, and other information. The Addendum can be downloaded from:
http://h2o.enr.state.ne.us/newetlands/appadd5.12df. Please refer to:
http://b2o.enr.state.nc.us/ncwetlands/regcert.html for design standards, worksheets, and sample
maintenance agreements.
Please respond within three (3) days of the date of this letter by sending five (5) sets of this information to
me in writing. The Express Review Program is a process that requires all parties to participate in a timely
manner. If the DWQ feels the Express Review Program is the incorrect review program for your project,
then we may have to return your application and have it reviewed through our regular 401 Water Quality
Certification Program.

or protected buffers. Please be aware that any impacts requested within your application are not
