Dear Ms. Karoly:
Mulkey Engineers & Consultants, Inc. (Mulkey) has been retained by Novartis Vaccines and
Diagnostics (Novartis) to prepare an annual report summarizing the inspection and maintenance
events associated with the south reinforced earthen slope structure from April 17, 2007 to May 12,
2008. The annual inspection reports are required by the North Carolina Division of Water Quality
(NCDWQ) Section 401 water quality certificate (DWQ Project # EXP 06-1376) issued for the
Novartis project on September 29, 2006. This report is meant to summarize observations, issues,
and maintenance conducted on a weekly basis since construction. Photos of the reinforced slope are
attached to this letter report.
A. Project Description
The south reinforced earthen slope is located on the southern side of the Novartis campus in Holly
Springs, North Carolina (Figures 1 and C JSK-8). It was built to minimize jurisdictional stream
impacts on the subject property where a vaccines manufacturing plant is currently under
construction. According to Qore Property Services (Qore), the slope was constructed utilizing
approved fill material placed in 9-inch lifts, with two rows of lifts comprising each row of metal
baskets (Figure 00-C-60-99-2). At the completion of each row, the top was dressed with topsoil to
ensure acceptable root growth from the sowed grass seed. This reinforced earthen slope was
completed and approved by Qore on April 3, 2007 (attached).
Weekly inspections of the reinforced earthen slope were conducted by Jacobs Engineering Qacobs)
beginning the week of April 16, 2007 and continuing to the present. These annual inspection reports
are required by the NCDWQ per special condition #8 in the water quality certificate issued on
September 29, 2006. This special condition requires that the structure be built per manufacturer's
guidelines and that inspections will occur (at a minimum) monthly for the first 6 six months and
quarterly thereafter for three years. Documentation during each inspection includes any structural
failings, significant mortality of cover vegetation, and descriptions of any repairs. NCDWQ has also
requested that photos taken at regular intervals be provided, along with the dates of all inspections.
B. Inspections
Below is an outline of minor issues that were observed and noted during the inspection events for
the past year according to inspection documentation provided by Jacobs. No issues were observed
and no significant rain events occurred during the weeks that are not referenced below.
May 21, 2007 - Additional matting was placed along the edges of the reinforced
slope up to the adjacent rip-rap channel.
June 3, 2007 - No issues were reported with the reinforced south slope following 1

'/2 inches of rain.
• The site received 3 1/4 -inches of rain on June 28 and 29, 2007 with no issues
concerning the reinforced slope reported.
• July 18, 2007 - The berm at the top of the slope washed out resulting in an erosion
rut down the side of the slope. These issues were the result of a 2 5/8-inch rain
event (measured onsite) on July 17. As a result the berm was immediately repaired,
the rut was filled and the area reseeded.
• July 31, 2007 - Rip rap was placed along both sides of the reinforced slope after a 1
1/4-inch rain event on July 30 created a few areas of erosion.
• September 17, 2007 - The site received a 2'/4 -inch rain event with no issues on the
reinforced south slope.
• Over four days between September 24 through 27, 2007 the site received 4 1/4 -
inches of rain; however, no issues were reported for the reinforced slope on
September 27 from these large rain events.
• December 17, 2007 - The site received 1 '/z-inches of rain with no issues concerning
the reinforced slope reported.
• December 31, 2007 - The site received 2-inches of rain with no issues concerning
the reinforced slope reported.
• March 4, 2008 - The site received 1 inch of rain with no issues concerning the
reinforced slope reported.
• The site received 1 '/2-inch of rain on March 7 and 8, 2008 with no issues
concerning the reinforced slope reported.
• April 7, 2008 - Repairs (including the placement of rip-rap and matting) were made
to an eroded area immediately east of the reinforced south slope. This area of
erosion was caused by the 1-inch of rain received on April 5 and 6.
• The site received 2-inches of rain on April 20 through 22, 2008 with no issues
concerning the reinforced'slope reported.
• 1-inch of rain was reported for both weeks beginning April 5 and April 12, 2008
with no issues concerning the reinforced slope reported.
Due to the severe drought conditions experienced throughout 2007, achieving adequate grass seed
growth for the purposes of erosion control was extremely difficult. Major conventional reseeding
efforts on the south reinforced slope occurred on July 27 and September 21. These were
unsuccessful in generating enough grass to adequately avoid potential erosion issues due to the
combination of a lack of rain and hot conditions. Therefore, Novartis employed the use of a cotton
fiber mulch placed atop the ungermanated grass seed to help hold the moist content longer. This
seeding occurred on October 8, 2007 and was successful; thereby, avoiding any additional reseedings.
C. Conclusions
The inspection data from April 2007 through April 2008 demonstrates that though there have been a
few minor erosion issues there have been no major failings of the south reinforced earthen slope
structure. All issues have been dealt with and attended to immediately following observation, as is
protocol for all erosion issues on the Novartis project site. If you have any questions concerning this

Representatives of QORE observed the construction of the south reinforced earth slope on a full-
time basis as the work progressed. Our scope of services entailed an evaluation of the soil
subgrades prior to fill placement, observation of the wire-basket placements, observation of the
geogrid placement, and in-place density testing of the fill materials. The structural fill materials were
generally placed in 9-inch lifts (two lifts per row of baskets) and the steps formed by the baskets
were topped with topsoil. Field reports were prepared on a daily basis as the work progressed and
these reports are submitted under separate cover.
In summary, it is our opinion that the work described above complies with the requirements of the
project Drawings and Specifications based on our professional judgment from field observations,
field tests, and limited laboratory tests conducted by QORE. One must not interpret our findings as
a guarantee of the entire work product.
