The project shall be constn�cted in accordance with your applicatien received pecember 1, 2015 and modification
reques± received September 27, 2016. After reviewina your application, v!�e have decided that these impacts are
covered by General Water Quality Certification Number 3891. This certification corresponds to the Nationwide
Permit 23 issued by the Corps of En�ineers. This approval is also valid for the Tar-Pamlico Riparian Buffer Rules
(15A NCAC .02B .0295). [n addition, you should acquire any other f deraL state or local permits befo:e you
proceed with your project including (but not limited to) Sediment anci Erosion Control, Nan-Discharge and Water
Supply Watershed regulations. This approval will expire with the accompanying 404 nerrnit.
This approval i� �alid solely fo� the purpose and design descri�ed in your application (un;ess rnod�fied below}.
Shoald your project change, you must notifj� the NCDWR and submit a new application. If the property is sold, the
new owner must be given a copy of this Certification and app� oval letter, and is thereby responsible for complying
with all the conditions. If total wetland fills for this project (now or in the future) exceed one acre, or of total
im}�acts to streams (now or in the fut�aie) ex��eed I SG linear feet, u�mpensatory mitigation n�ay be reyuired as

1. All conditions in the Original Water Quality Certification for this project, dated December 30, 2015, still apply.
If you wish to contest any statement in the attached Certification you must file a petition for an administrative
hea.ring. You may obtain :he petiticn form from the otiice of Administrative hearings. You :nust fi!e the petition
with the office of Administrative Hearings within sixty (60) days of receipt of this notice. A petition is considered
filed when it is received in the office of Administrative Hearings during normal office hours. The Office of
Administrative Hearings accepts filings Monday through Frid«y between the hours of 8:OUam and S:OUpm, except
for official state holidays. The origina[ and or�e (1) copy of the petition must be filed with the Office of
Administrative Hearings.

Administrative Hearings within five (5) business days following the faxed transmission.
