Resident Engineer, Scott Wyrick- Lead Project Inspector NCDOT, Amy Euliss - DEO, Scott Gallman (Dragados) Follow up inspection for the ICA report issued on 8/5/2016. The ICA status can be lifted at this time. Project work has been suspended by the Resident. Additional rainfall has hampered some repair work, but crews are continuing the cleanup and installation of the EC devices per the ICA report. -Crews are refurbishing drop inlet protection along the toe of slope approaching Site 10 after rains subsequent to the sediment loss. -Discussed the EC for the roadway section approaching site 10 to clarify what is required by the 8/5 ICA report. Define the TSD parallel to the perimeter berm and install silt checks per the ICA report. Extend the diversion out across the grade to direct runoff to silt checks between slope drains. This applies to both right and left of L- line. -Discussed slope drain inlet protection and the need to keep the inlet of the slope drain open with small amount of protection to prevent scour. Insure berm over and around the slope drain is well compacted to prevent undermining of the drain. -Per previous reports and discussions, install two large Type A sediment dams above both drop inlets along the rock lined ditch. Weir height should be at least 3' high. -Along the haul road to the field office, reinstall silt checks and type b silt basins to handle slope drain discharge. Clean out skimmer basin at pipe inlet. Con Remove all excavated and accumulated sediment from the basins and ditch and remove it from the area. -Continue cleanup of skimmer basins both right and left side of the project. -Met with crew leaders and foreman onsite to discuss the EC principles and site application for this work. Emphasized the need for 7 0 8/05/2016 1:32 PM Page 1 of 2 Level II supervisors and crew foreman to direct EC work and be responsible to see measures repaired and installed. Contractor should bring this roadway section to a static grade, install the EC devices per report and onsite discussions. Seed/mulch this section to stabilize it and reduce the erodible area. All "UrgenY' items on the NPDES list need to be addressed. 8/05/2016 1:32 PM Page 2 of 2
