Dear Mr. Roberts:
A review of the August 2014 self-monitoring reports for the subject facility revealed a violation of the
following parameter at Outfall 001:

Week ending 812/14 BOD None Monitor once per week
Remedial actions, if not already implemented, should be taken to correct the cause of the noted
violation. Unresolved violations may lead to the issuance of a Notice of Violation and/or assessments
of civil penalties by the Division of Water Resources.
If the violation is due to a transcription error(data left off the DMR) please submit an amended DMR
(two copies) with the relevant quarterly monitoring data to this Office along with a cover letter asking
for rescission of the Notice of Deficiency. Please address your correspondence to Ms. Marcia Allocco
