Dear Mr.Finley:
The Division of Water Resources(DWR or the Division)is concerned about your late submittal Oreco d monthly
Discharge Monitoring Reports(DMRs)for the subject wastewater treatment plant(WWTP).
Yourindicates that we have not received a DMR report for October 2015.
Failure to submit a timely DMR subjects you to civil penalty assessments not to exceed$25,000 per
violation.If this facility is not or was not discharging,you are still required to submit a monthly DMR
indicating"no discharge"or"no flow."In accord with the NPDES Program,DMRs are due no later than
the last day of the month following the month of reporting,i.e.,a June report is due no later than July 31
[Part II,Condition D(2); 15A NCAC 2B.506(a)].
Please provide the missing documents by May 2,2016. If you have records documenting that this report
was submitted to the Division before the required deadline,please forward.
Please note that a repeat violation for late reporting within the calendar year May 2016—April 2017 will
prompt this office to assess a civil penalty.We appreciate that you will forward this letter to whomever else
