The Department applied for a permit for this project on February 4, 2016. The NCDENR-DWR issued certifications for
the project on March 4, 2016. Two construction issues have come up at this time. Note the following:
Issue at Site 1(Bridge near 21+00 -L-)
A portion of the project limits at the bridge is located within the Smith Austin Creek Mitigation Site and this use requires
coordination with the North Carolina Interagency Review Team. An existing rock cross vane installed as part of the
mitigation site is located directly at the outlet of proposed standard v ditches upstream of the bridge. The Department
proposes "field-set and integrate" the ditches to the existing rock cross vane. Specifically, the ditches will be shifted as
necessary to be slightly upstream of the top of the rock cross vane to provide at least 2 feet of separation between the
low point of the proposed ditches and the top of the rock cross vane. This solution will integrate the proposed ditches
and rock vane together, creating the most stable long-term solution.

The fill on the right shoulder of the road (downstream side of the culvert) exceeds the maximum allowable of 5.0 feet
for the aluminum box culvert per AASHTO specifications. We were under the understanding this 5.0' max cover applied
under the roadway, but according to Pomona Pipes (delegate designer of the culvert) it also applies on the shoulder. As
currently designed there is approximately 5.7' of fill at the right shoulder point.
To reduce the fill, we are proposing to raise the downstream invert 1 foot to the natural channel bed elevation. The
invert at the outlet will be set to match the adjacent downstream elevation. A mix of Class B and Class I rip rap will be
installed in the a slight blow hole (i.e. countersunk) in the streambed below the outlet. The upstream invert would
remain buried 1 foot and a one foot sill would be placed at the upstream face (slightly inset inside the culvert). A mix of
Class B and Class I rip rap will be installed in the streambed (i.e. countersunk) up to the top of sill. The countersunk rip
rap in the streambed at the inlet and outlet of the culvert is fully within the previously permitted stream improvements.
