Remarks and Recommendations:
Reviewed project today with Jeremy Guy, Lindsey Panek, Brian Reese and Amy Euliss.
Project impacted by heavy thunderstorms yesterday. There are urgent erosion control repairs that remain from
the last report and more that have occurred since this rain. Any direction for cleanup of sediment by Amy
Euliss, DEO, and by DWR if they conduct a site review, shall be done quickly to prevent further impacts.
-Sta 582+60 to 575+00, slope drain spacing was improved. Insure the drains are long enough to reach the toe
of the slope and provide energy dissiaptor at the outlet.
Slope drain inlet protection should not completely block the slope drain. At approx sta 568, this caused a slope
failure due to poor installation and sediment loss into Site 10 along the toe of slope. Repair the slope and
berm and lower the rock around the slope drain pipe.
All slope drains should be reviewed to make sure they are functioning.
-Reinstall the large Type A Rcok silt check in the ditchline above the big drop inlet approx sta 563. This is to
trap any silt moving down that ditchline from the slope drains. It was approved instead of a basin in that area
and should be reinstalled and maintained.
As the grade is worked it should be crowned to drain to either side as evenly as possible in the future.
-Tabors Pond area: Install a wrapped TRSC-A with PAM application above basin ID 8.2-B to help control
turbidity.
Along the section over Tabors Pond, direct runoff to Basin 8.3, limiting the drainage area to 1 acre. Direct
roadway runoff to both sides of the project, all four corners of the site. Utilize raised berms across the rock fill if
needed to help shift storm water flow so it all does not go to the right side.
Regrade and compact the berm along the right side of the roadway to prevent runoff from breaking over the
top. Grade the roadbed to drain to all four corners of the site so that no standing water is left along the
roadway over Tabors Tond. Seed and mulch the roadway area, allowing only two lanes in the center for traffic.
-Site 6, left of L, cleanup accumulated silt behind the silt fence. Restabilize the toe of slope with seeding and
matting. Install slope drain per plans and direct it to Basin 7.4B.

Contractor needs to address EC needs as noted by NCDOT personnel and Roadside Environmental reports in
a timely matter. Each crew should have Level I installer and Level II foreman per the standard provision.
Some of the repair work done does not function properly or meet specs so this should be reviewed by the
Level II and DOT inpectors before it is approved.
A follow up inspecdtion wil Ibe conducted by Wednesday, May 18 to determine the progress of the repairs from
this report and the previous report issued on 5/3/2016.
