Attached is the erosion control report from the inspection that was conducted yesterday. The project did receive a lot
of rain the last few days but the report also indicates the overall lack of ensuring the project was sufficiently prepared
for this rain event. This report along with the NPDES “Routine” and “Urgent” repair and maintenance list need to be
corrected as soon as project conditions allow. The off-site sedimentation in the stream at site #10 needs to be cleaned-
up as soon as possible and the cause for the loss of silt needs to be corrected. Per the NCDENR directive the stream
needs to be cleaned from the end of the silt deposit and work your way up towards the source of the silt. Failure to
abide by this directive will result in further corrective requirements.
So starting this morning all work on the project that utilize roadway personnel is to cease until all erosion control
measures are repaired or corrected and department personnel have a chance to review the corrective
work. Subcontractors are allowed to work as long they do not require assistance from roadway personnel that should
be performing erosion control work.
The cut slopes in the borrow pit will need to be corrected to a 2:1 or flatter slope, seeded and mulched before
the hauling operation can resume.
The areas where the clearing contractor has cut trees along Business 40 will need to be properly grubbed prior to any
more trees being cleared.

Project impacted by recent heavy thunderstorms. There are urgent erosion control repairs that need to be
made. Additional EC devices need to be installed per this review and previous site reviews by NCDOT
personnel. Any direction for cleanup of sediment by Amy Euliss, DEO, and by DWR when they conduct their
site review, shall be done quickly to prevent further impacts.
-Sta 582+60 to 575+00, insufficient slope drain spacing along the right of the project. This area was noted in
reports and by NCDOT inspectors as needing additional slope drains. Max spacing is 200' so additional drains
are needed to meet this at a minimum.
As the grade is worked it should be crowned to drain to either side as evenly as possible in the future.
-Contractor is walking entire slopes when stage seeding and tearing up whole slope face. Slope should be
walked only to dress up newer lifts and preserve existing permanent ground cover on lower portion of slope.
-Slope drains should take direct paths to basins. This will shorten lengths and be less likely to separate.
-In Smith Creek area, TSD into basin 15.10 failed and runoff bypassed basin. Cleanout behind silt checks in
old creek channel. TSD's should have well compacted berms to help stabilize them. Repair the TSD's and
clean out accumulated sediment at the rear of the basin to help keep it open for storm water flow.
-Basin 27.4, runoff bypassing basin and undermining silt fence. Repair and maintain TSD's to direct flow to the
basin.
-Insufficient inlet protection at inlet of 54" pipe. Add large rock inlet sediment trap at inlet and an additional
large TRSC-A in the ditch. Recommend a compacted berm to surround and close off this ditch from
surrounding erodible slopes. Add slope drains to control runoff and discharge it behind the silt check.
Remove accumulated sediment from pipe outlet and energy dissipator. ASAP finish out pipe outlet rock lined
ditch.
-Dress up slopes to 2:1 or flatter and seed slopes in the Peddycord borrow pit that are not going to be
excavated further. These areas exceeds the time frame for establishing groundcover.
-Westingbrook Trail, left of L, silt fence over the creek needs to be cleaned out and haul road mucked out.

Regrade the haul road to prevent ponding over the creek. Stabilize the haul road over the creek to prevent
erosion and further sediment loss. Recommend possible use of concrete barrier either side to contain the
haul road.
Construction entrance pad needed to prevent tracking the haul road mud up onto Westingbrook Trail.
-At Site 31, concrete waste not contained in washout area. Deposited right behind silt fence near creek.
Concrete waste is required to be contained at established washout areas.
Erodible slopes along haul road on project right need to be stabilized with groundcover. These exceed
seeding limit requirements.
Clean off temp crossing bridge.
-Site 10, clean out accumulated sediment in pools, on sides and in the channel bottom per Amy's comments.
Baffles installed in the channel to help slow movement of silt. Clean off baffles.
Clean out the pipe system to remove accumulated silt from the slope drain failure noted above.
-Basin 10.10, clean out silt fence at wing wall, reinstall and stabilize the slope. Recommend adding rip rap up
around the wing wall to help stop erosion if too steep for matting.
-Basin 5.16 C- Stabilize the outlet with geotextile and rip rap where it's eroded out. Recommend remove silt
fence along tie of slope Y1 detour.
-Clean up offsite sediment at outlet Y1 right, sta 42.
-Clean out skimmer basin 5.15
-Contractor dewatering for bent a bridge 1 at Y1, approx sta 35, right, extend hose to grass; water is clean so
sending it across erodible areas just causes unnecessary erosion
-Site 1, clean accumulated silt off energy dissipator .
-Where Smith Rowe has cleared and grubbed along Y4 there is excessive erodible area. Provide temp
groundcover. Several areas have undermined silt fence into jurisdictional spots. Reset silt fence and use
TSD's with compacted berms to direct runoff to special sediment control fence sections or other available
devices.
Contractor needs to address EC needs as noted by NCDOT personnel and Roadside Environmental reports in
a timely matter.
Proper installation of additional devices as needed and directed by inspectors is critical to prevent erosion and
sediment loss.
