JAY ZIMMERMAN Urreclor RECENEDINCDE0]D\1VR APR 13 2016 Water Quality ecion Permitting A Compliance Evaluation Inspection was performed on the facility by George Smith, along with April Norton on March 23, 2016. Mr. Brandon Hughes, ORC, Grade II; Mr. Jadd Brewer, Back-up ORC, Grade III; were present for the inspection. The inspection is comprised of an overall evaluation of the facilities equipment, capability to meet effluent limits, and compliance with the NPDES permit. Permit Your new NPDES permit became effective October 1, 2012 and expires September 30, 2017. This office received a copy of the letter dated February 17, 2016 requesting the facility be changed from composite to grab sampling, since the permitted flow is 0.03 MGD. Records & Reports The operator and plant log are printed on a form showing visitation times and plant conditions. The log will be examined more thoroughly during the next year inspection for visitation times, observations, and process control. The chain of custody forms and calibrations records are in good order. Calibrations for field tests were documented for pH, Temperature and Dissolved Oxygen. Facility Site Review The plant is adjacent to a golf course tee box. A new fence is being installed. A gravel base has been placed around the treatment units. The manual 'A inch barscreen is acceptable. The equalization basin blower motor was being repaired There are two in -ground aeration basins. The basin on the right side is not used. The course bubble diffusers are delivering a good rolling action. The mixed liquor appeared light brown color with less than 10% of the surface of the basin covered with tan foam. Also, lime is added to the plant. a At the time of the inspection there was a lot of laundry detergent entering the plant. Mr. Brewer indicated that there are no adverse impacts. The clarifier effluent was clear. The Trojan 2000 UV disinfection unit is working. The indicator lights on the UV were not illuminated. The previous inspection showed that three of the four lights were illuminated. Flow Measurement Continuous flow measurement at the plant is performed using a 22 '/2 ° V -notch weir with an ultrasonic device mounted before the effluent weir. The flow measuring device was calibrated by B.W. Allen on October 8, 2015. Laboratory Water Quality Lab, Inc. #544 is certified, and performed the following field tests: Dissolved Oxygen, pH, and temperature. The pH meter is calibrated with a two-point buffer of 4 & 10, and check with a pH 7 buffer. A calibration log was kept for pH and dissolved oxygen. Temperature is traceable to NIST. Effluent/Receiving Waters The effluent, which discharges to Laurel Fork, Class C Trout waters, was clear and had no color, solids, or odor. The stream is not impacted from this discharge. Operations & Maintenance The Class II facility was maintained by the following contingent: Mr. Brandon Hughes, ORC, Grade II and Mr. Jadd Brewer, Back-up ORC, Grade I1I. Self -Monitoring Program The effluent is sampled using a time composite method at 100 mL/hour. The NPDES requirement for flow proportion sampling will not be addressed at this time. The NPDES Unit is reviewing the request to change the permit from flow proportional sample to grab sample. A review of the monthly self-monitoring reports from August 2015 through January -2016 showed no limit violations. The compliance inspection is considered satisfactory.
