In accordance with your application for a Certificate of Coverage [received on November 16,2004],the Division
is forwarding herewith Certificate of Coverage NCG550308 to discharge under NCG550000. Your residence previously
was assigned this CoC number in 1993. The previous owner allowed the CoC to expire. Thus your application is being
treated as a renewal of an existing CoC,rather than a new discharge application. This permit is issued pursuant to the
requirements of North Carolina General Statue 143-215 .1 and the Memorandum of Agreement between North Carolina and
the US Environmental Protection agency dated May 9, 1994 [or as subsequently amended].
If any parts,measurement frequencies or sampling requirements contained in this general permit are unacceptable
to you,you have the right to request an individual permit by submitting an individual permit application.Unless such
demand is made,the certificate of coverage shall be fmal and binding.
Please take notice that this Certificate of Coverage is not transferable except after notice to the Division. The
Division may require modification or revocation and reissuance of the certificate of coverage.
This permit does not affect the legal requirements to obtain other permits which may be required by the Division of
Water Quality or permits required by the Division of Land Resources,Coastal Area Management Act or any other Federal
or Local governmental permit that may be required.

Charles I received your letter to Lafayette Davis concerning renewal .
I called and spoke with Julia Williams Davis . She is the mother of
Lafayette. Lafayette is now in Phoenix Arizona. Permit should be in
the name of Julia Williams Davis . If possible send her another
verify name address and discharge. Then she could
renewal form to ve y g
make the name change on the form. I believe Ms. Julia is close to city
sewer. My approach Charles since coming to the region is to get them
all tied in to city sewer. Where sewer is not available I encourage
them to petition. I did give her the info and number to call at
Durham City in reference to sewer connection (Water Management, Water
and Sewer Maintenance-919-560-4344 . I did tell her to give you a call
about the renewal . She said that she had a sandfilter discharge. Keep
this in mind. Before the next batch of renewal letters go out for
NCG55 in 2007 please let me take a look at the letter and the permit.
There are some ideas I have in mind to add. Thanks . Mack
