Completed
Due to a fatality at this site last year, this project has been redesigned to improve traffic safety. A vehicle with a trailer
fell over into the canal last year while turning at the intersection, and the driver drowned in the canal. To improve
traffic safety, the culvert has been extended to widen the shoulder and to extend the turn radius at the intersection.
Attached are revised permit drawings and PCN for the project. NCDOT requests to modify/reissue the permit and 401
Cert. for this project. Please let me know if you need any additional information or have any concerns.

The above project qualifies for Nationwide Permit 3 authorization. This permit authorizes replacement of two
existing 84" by 60' and one 84" by 68' corrugated metal pipes with one 18' by 11'4" by 59'9" aluminum arch culvert in
Main Canal under B Canal Road east of Roper in Washington County, North Carolina.. Please see the attached
authorization and conditions for NWP #3. As stated in the permit conditions, the new culverts must be buried 12 inches
below the substrate.

Project area consist of NCDOT Washington County SR 1180 with Main Canal passing under the road through 2@
84"X60' and 1@84"X68' metal culverts. The adjacent property use is a mix of agricultural and forested area.

3d. Explain the purpose of the proposed project:
Replace deteriorating metal culverts and maintain a stabile road way along SR 1180 for the safety of the traveling public.
3e. Describe the overall project in detail, including the type of equipment to be used:
The existing 3 metal pipes will be entirely removed using NCDOT guidelines for pipe removal. A new aluminum arch culvert
pipe measuring 18' X 11'4" X 73'3" with headwalls will be placed as shown in the provided permit drawings. The new arch
pipe will be buried 1 foot below the surveyed stream bed. Once the pipe is in place and back filled a new roadway surface and
grassed shoulder will be established. The headwall tie in points will likely need some rip-rap stabilization as shown on the
permit drawings. Some temporary disturbance to the vegetated banks will likely occur from the dewatering process and
headwall installation. These banks will be stabilized following NCDOT erosion control bmp's. This project will result in 607 sq.
ft. of open water impacts due to the longer pipe and widening of the shoulder. This work will be accomplished using a track
excavator, backhoe, and dum trucks.

5/7/2015. Due to a traffic accidient involving an overturned vehicle in the canal resulting in a fatality at the site, the project
was redesigned to increase the shoulder area to improve the radius of the curve of the intersection. This redesign will
imrpove the safety of traffic turning at this interstion.

Impacts have been minimized by utilizing an arch culvert and headwalls that reduces the footprint along the tributary bottom.
