Marshall and Mary Lynn Noyes, Owners Residential Wastewater Treatment System Cabarrus County Dear Mr. Noyes: A letter of request for Authorization to Construct was received April 9, 1984, by the Division and final plans and specifications for the subject project have been reviewed and found to be satisfactory. Authorization is hereby granted for th e construction of a 300 GPDwastewater treatment facility consisiting of a 1,500 gallon baffled septic tank; a 1,500 gallon storage and pump tank with recycle system, on-off timer controls, a high water alarm system, and a 24 GPM rump; a subsurface sand filter, a tablet type chlorinator; an 800 gallon chlorination tank with aeration; a sampling box; collection and connection piping; fittings; valves; electrical services and appurtenances to serve the two (2) bedroom Noyes residence on Lot 19, Lake Shores Estates. This Authorization to Construct is issued in accordance with Part III paragraph C of NPDES Permit No. NC0059072 issues April 24, 1984, and shall be subject to revocation unless the wastewater treatment facilities are const ructed in accordance with the conditions and limitations specified in Permit No. NC0059072. The sludge generated from these treatment facilities must be disposed of in accordance with G. S. 143-215.1 and in a manner approvable by the North Carolina Division of Environmental Management. The Mooresville Regional Office, telephone number 704/663-1699 shall be notified at least twenty-four (24) hours in advance of backfilling of the installed subsurface filter system so that an in-place inspection can be made of said system prior to backfilling. Such notification to the Regional Supervisor shall be made during the normal office hours from 8:00 A.M. until 5:00 P.M. on Monday through Friday, excluding State Holidays. In event the facilities fail to perform satisfactorily in meeting it's NPDES permit effluent limits, H. Marshall and/or Mary Lynn Noyes shall take such immediate corrective action as may be required by this Division, including the construction )f additional wastewater treatment and disposal facilities.
