I hope all is well. I am currently processing the application referenced in the subject line above.
During the technical review period, additional information regarding your Fast Track Sewer
Extension permit application is warranted. As of the date of this email, the technical review
period (45 days) is paused until we receive the additional information requested. Additional
information must be received within 30 days, or the application will be returned to the applicant.
Please clarify/correct the following:

1. The application indicates that a 401/404 permit is required. Please provide a copy of the
permit(s).
2. The submitted USGS map is insufficient. Please provide a new USGS topographic map that
clearly identifies the project area.
Please note that you do NOT have to mail the requested items. You may respond directly to me
via email.
