It appears that an error was made when submitting the revised Supplement-EZ form.The digital and hard copy of the form that was provided has two identical Wet Pond pages, and no Low Density page. Please submit the Low Density page of the Supplement, and ensure that it is consistent with what is shown in the provided calculations. Response: Corrected Supplement-EZ Form is provided with this submittal. 2. From the provided plan sheets, it seems as though there was an error with correcting the forebay's grading. Per Wet Pond MDC 5,the forebay entrance shall be deeper than the forebay exit.This refers to the excavated bottom of the forebay, not the sediment storage height. Both the provided plan sheet and the O&M Agreement indicate that the sediment storage elevation will be lower than the forebay bottom, and the sediment storage appears to be graded with the intent to meet the MDC, not the excavated bottom of the forebay. Please revise the project package to meet the MDC, and ensure that all relevant documentation (the Supplement-EZ and the O&M Agreement) is consistent with what is shown on the plan sheets.
