Authorization is hereby granted for the construction of modifications to the existing 1.2 MGD Town of Weldon WWTP, with discharge of treated wastewater into the Roanoke River in the Roanoke River Basin. This Authorization to Construct is issued in accordance with Part III, Paragraph A of NPDES Permit No. NC0025721 issued effective August 1,2012, and shall be subject to revocation unless the wastewater treatment facilities are constructed in accordance with the conditions and limitations specified in Permit No. NC0025721. I The Raleigh Regional Office, telephone number (919) 791 - 4200, shall be notified at least forty-eight (48) hours in advance of operation of the installed facilities so that an on site inspection can be made. Such notification to the regional supervisor shall be made during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through Friday, excluding State Holidays. Mayor Julia M. Meacham January 3, 2017 Page 2 of 3 Upon completion of construction and prior to operation of this permitted facility, a certification must be received from a professional engineer certifying that the permitted facility has been installed in accordance with the NPDES Permit, this Authorization to Construct and the approved plans and specifications. Mail the Certification to: Division of Water Resources, WQ Permitting Section - NPDES, 1617 Mail Service Center, Raleigh, NC 27699-1617. Upon classification of the facility by the Certification Commission, the Permittee shall employ a certified wastewater treatment plant operator to be in responsible charge (ORC) of the wastewater treatment facilities. The operator must hold a certificate of the type and grade at least equivalent to or greater than the classification assigned to the wastewater treatment facilities by the Certification Commission. The Permittee must also employ a certified back-up operator of the appropriate type and grade to comply with the conditions of T15A:8G.O2O2. The ORC of the facility must visit each Class I facility at least weekly and each Class II, III and IV facility at least daily, excluding weekends and holidays, must properly manage the facility, must document daily operation and maintenance of the facility, and must comply with all other conditions of T15A:8G.O2O2. During the construction of the proposed additions/modifications, the permittee shall continue to properly maintain and operate the existing wastewater treatment facilities at all times, and in such a manner, as necessary to comply with the effluent limits specified in the NPDES Permit. Prior to entering into any contract(s) for construction, the recipient must have obtained all applicable permits from the State. Failure to abide by the requirements contained in this Authorization to Construct may subject the Permittee to an enforcement action by the Division in accordance with North Carolina General Statute 143-215.6A to 143-215.6C. Attached is a narrative of the events/actions taken upon discovery of the sludge spill at the Town of Weldon's WWTP. 4/19/17 Incident Number 201700546 - Town of Weldon Sludge Spill Mr. Rimmer, Thank you. On April 17, 2017 at 8:00AM, Bill Hux and Bobby Lyons (back-up ORCs/operators) arrived at the Town of Weldon WWTP. Upon completing morning rounds, at 8:30AM Bill Hux noticed/found the above ground sludge storage tank had leaked roughly 6,000 gallons of sludge on the ground. The gallons were determined based off the volume of the tank in inches and the number of inches the sludge level dropped inside the tank. The leak was caused by a small rusty spot that appeared to be surface rust. A wad of rags initially seal the hole. Bill made a wooden plug and drove it into the hole temporarily until a permanent repair could be made. At 8:53AM Bill called me to advise me of what he had discovered and told me the actions he had taken. I arrived at the WWTP at 9:10AM. A local industrial cleaning company (MPW) with a vacuum truck was contacted to assist with the clean-up effort. The sludge was spilled on the ground between the sludge tank and the sludge drying beds. The sludge was/had stabilized however bales of hay were placed around the perimeter to prevent any further movement. Upon being advised that MPW was unable to arrive until Tuesday 4/17/17 the town utilized our tractor/ffontend loader to scoop the sludge off the ground and place it on the sludge drying beds. Once the sludge was removed, lime was spread over the affected area. A toggle anchor with rubber gasket and washers was installed to replace the wooden plug. I contacted you Monday (4/17/17) at 1:23PM to report this spill. Donald L. Bill’s last visit to the WWTP on Sunday was at 5:00PM.
