DWR has conducted a review of the above noted application and requires additional information to complete our review. Please respond to the items listed below within 30 days of receipt
of this request. Please note that the application will be considered “on hold” until receipt of a complete response.

1. Provide details on how the temporary impacts will be restored to pre-existing conditions. Include a re-planting plan with details on seed mix, species, live-stakes, grading, etc.

2. Add shading to the impact maps to show where the temporary impacts are. It’s hard to tell their extent in context to the culvert from the arrows on the maps. [15A NCAC 02H .0506(b)(1)]

3. From the sheets labeled “Impact Area 1 Sections” and “Impact Area 2 Sections,” explain how the upstream and downstream re-grading fits into the impacts. [15A NCAC 02H .0506(b)(1)]

4. Explain why the proposed grade (highlighted on the image below) is at that angle/shape? [15A NCAC 02H .0502(c)]
