and corresponding index number. Please enter only immediate receiving waterbodies. Ensure you are
not entering the river basin information. The correct waterbody index number will not contain any letters.
B1. This permittee does not match the permittee that the letter of approval/grading permit is addressed
to.
B1 /D6/D9/E. The entity listed on B1 must match the responsible entity/permittee listed on the FRO form,
NOI Certification form (uploaded to D9), and Tab E (certification tab).
B2&B3: The person listed on blocks B2&B3 must match the responsible person listed on the NOI
certification form and Tab E.
D2. Please change this to the project ID listed on your approval letter.
