This email is to notify you that your application for Oaks at Whitaker Glen was not accepted forth e
following reason:
B1 /D6/D9/E. The entity listed on B1 must match the responsible entity/permittee listed on the FRO form,
NOI Certification form (uploaded to D9), and Tab E (certification tab).
D1. Ensure this date matches the issue date of your grading permit.
D2. This should be the permit number of your grading permit.
D5. Please attach only the grading permit associated with the project you are applying for NCG01
coverage for. There are 2 grading permits attached for the same location with different amounts of
approved disturbed acreage.
D9. The boxes on the NOI Certification Form must be checked to indicate agreement.
E. The person that certifies this form must be the same person listed as the responsible person (Tab 132)
and on NOI Certification Form (Tab D9). If signing as authorized responsible person, that person must
also sign the NOI certification form.
Please submit a new NOI. To auto -populate the new NOI, select'Yes' for the first question & enter NOI
