August 14, Yes Thank You for Using Online Portal consistent with current Division of Energy, Stormwater
2024 11:51 Mineral,and Land Resources Monitoring Form Rev.07012020 under your Program
responsibility, Division of Energy, Mineral,and Resources Form Rev 01272021 and Manager-
consistent with On Site Observations in reference to a received plan on May 26, Seneres
2023 and approved under COA-2023-010 dated May 25,2023 and Certificate of
Coverage NCG0106022023-0602 NCC 231695 dated May 30,2023 located in WS-IV
17-9-(1)HUC 030300030106 with a noted longitude and latitude. On Site
Observations observed by a Verified Official as defined by See Click Fix Protocol.
In addition,the site being in the jurisdiction of Randolph County and defined by
the noted location generated by geofencing with latitude and longitude located
in the headwaters of the defined river basin with a 911 project designation with
the person financially responsible verified on the authorized Certificate of
Approval.The site currently is not under notice of violation and no notations to
subparagraphs of a standard North Carolina Sedimentation Report are warranted
at this time.Any documentation,observations,and photographic are included
with additional comments if needed As evidenced by the photographic
documentation, no significant violations have been observed using paragraphs
5a through 51 not used unless noted and paragraphs 6a through 6v through site
observations of a verified official.
The following conditions have been met for final closeout of the project listed on
the Certificate of Plan Approval issued by the City of Archdale under the
Authorization of the ACE Integrated Stormwater Management Progam. 1)The
Standard Stormwater Operations and Maintenance Agreement has been duly
signed and filed with the Register of Deeds.2)The City of Archdale has received a
digital version of the CCTV of subsurface Stormwater systems.3)Tracing of
underground systems has been verified by Public Works personnel;and
permanent ground cover sufficient to restrain erosion has been established. No
additional self-monitoring or self-inspections are required. Project is in
compliance and released under the SPCA.Termination of coverage under the
NPDES General Stormwater Permit for Construction Activities(NCG01)must be
requested by the permittee via a Notice of Termination(NOT).The NOT is
available at: https://edocs.deq.nc.gov/Forms/NCG01-Termination. Please note
that this correspondence must be uploaded as part of completing the NOT
process.
