Source of the Upset/Spill/Bypass (Location or Treatment Unit): The spill occurred during a backwash when
the backwash waste pit overflowed, which is prior to the chlorine contact chamber. The water in the pit was
backwash flow from the chlorine contact chamber which is used to backwash filters.
Cause or Reason for the Upset/Spill/Bypass: Mechanical pump failure.
Describe the Repairs Made or Actions Taken: The motor will be removed and sent off for repair.

Action Taken to Contain Spill, Clean Up and Remediate the Site (if applicable): Most of the water had
soaked into the ground when I arrived to estimate spill volume, any areas still standing will be limed.
Action Taken or Proposed to be Taken to Prevent Occurrences: Once the motor is repaired and pump is
back in service, will get recommendation from repair shop.
