Below is a snapshot of the Tier Three requirements from the NCG14 permit. Please note the requirements and deadlines from the permit. Please contact us if you have questions. Table 5: Tier Three Response for Four Benchmark Exceedances Within Five Years Timeline From Receipt of Fourth Tier Three Required Response/Action Sampling Result Continuously i. Document the exceedances and each required response/action in the SWPPP in accordance with E-7(c) above. ii. Continue monthly monitoring for all parameters at the subject outfall and continue until three samples in a row are below the benchmark value. Within two weeks iii. Notify DEMLR's Regional Office in writing of the affected outfall, four exceedance dates and values. iv. Conduct a stormwater management inspection. v. Identify and evaluate possible causes of the benchmark exceedance. Within one month vi. Prepare an Action Plan that should include specific, feasible courses of action to reduce concentrations of the parameter(s) of concern including, but not limited to,source controls, operational controls, or physical improvements and submit to DEMLR's Regional Office for review and approval. Upon DEQ Approval vii. Implement the approved Action Plan. Upon Completion of vii. Notify DEMLR's Regional Office of Action Plan completion. Approved Action Plan Jerry W. (Tri State), please find attached results of stormwater outfall sampling that was conducted on June 27, 2024, for the Tri State Gunite facility in Monroe, Union County, North Carolina. Storm water samples were collected from outfall SDO-1 by GRI personnel on June 27, 2024, in accordance with the sampling requirements of the permit. Water samples were submitted for analysis of total suspended solids (TSS) and non-polar oil and grease. The pH reading was measured in the field. The June 2024 sampling results indicated that the TSS levels exceeded the permit requirements, and the measured pH was outside the allowable range of the permit in the sampled outfall. In accordance with the permit, the pH and TSS exceedances trigger a Tier II response that require the facility to address the exceedances by making site improvements and to conduct monthly sampling events to confirm compliance of pH and TSS levels with permit requirements. The Stormwater Discharge Outfall Monitoring Report is included in Appendix A, the Stormwater Discharge Outfall Qualitative Monitoring Report is included in Appendix B, and a complete laboratory analytical report is included as Appendix C. 3502 Hayes Road • Monroe, North Carolina 28110 113 West Firetower Road, Suite G • Winterville, North Carolina 28590 Phone (704) 845-4010 • (888) 870-4133 • Fax (704) 845-4012 Discharge Monitoring Report—April-June 2024 (Second Quarterly Period of 2024) Tri State Gunite of NJ, L.P.- Monroe Facility Monroe, Union County, North Carolina Certificate of Coverage# NCG140505 Page 2 of 2 The next quarterly sampling event is scheduled to be conducted between July and September 2024 during a representative rainfall event.
