activity listed on the FRO form, NOI Certification form (uploaded to D9), and Tab E (certification tab).
B2&B3: The person listed on blocks B2&B3 must match the responsible person listed on the NOI
certification form and Tab E.
D5. The attached grading permit is failing to load. Please ensure the document is correctly
uploaded/formatted and resubmit.
E. The person that certifies this form must be the same person listed as the responsible person (Tab 132)
and on NOI Certification Form (Tab D9). If signing as authorized responsible person, that person must
